围绝经期情绪障碍对女性生活质量的影响

目的探讨围绝经期女性的情绪障碍对生活质量的影响。方法以203例围绝经期女性为研究对象。根据诊断标准将人群分为四组：对照组、围绝经期综合征组、情绪障碍组、围绝经期综合征合并情绪障碍组,对所有研究对象进行改良Kupperman绝经指数量表（KMI）、汉密尔顿焦虑、抑郁量表（HAMA、HAMD）、症状自评量表（SCL-90）、生活质量综合评定问卷中（GQOLI-74）的生活质量总评、社会功能维度量表的评估。结果情绪障碍组和围绝经期综合征合并情绪障碍组的社会功能和生活质量评分值显著低于对照组（P〈0.05）,情绪障碍组的分值分别为：（55.00±11.83）分、（10.67±2.18）分;围绝经期综合征合并情绪障碍组的分值分别为：（52.58±13.83）分、（9.90±1.97）分。而围绝经期综合征组的生活质量两个维度的评分与对照组相比差异无统计学意义,与围绝经期综合征伴有情绪障碍组比较差异有统计学意义（P〈0.05）：（60.16±12.51）分vs（52.58±13.83）分;（12.79±2.38）分vs（9.90±1.97）分。围绝经期症状中情绪症状、躯体症状、性生活均与社会功能和生活质量呈负相关,其中情绪症状的相关更为显著（P〈0．01），进一步回归分析后明确在围绝经期，抑郁和睡眠障碍是影响生活质量的主要因素。结论围绝经期人群的情绪障碍比躯体症状对社会功能和生活质量的影响更为显著。

Effects of Mood Disturbance on Quality of Life in Perimenopause Women

Objective To explore the effects of the mood disturbance on the quality of life in perimenopause women.Methods 203 women（48.13±4.91 years old） were included in this cross-sectional study,and were divided into 4 groups：normal group（n=44）,perimenopause syndrome group（n=33）,mood disturbance group（n=52）,and perimenopause syndrome with mood disturbance group（n=74）.All subjects were assessed using KMI,HAMA,HAMD,SCL-90,general comment of quality of life domain and social function domain of GQOLI-74.Results The scores of general comment of quality of life domain and social function domain were lower in mood disturbance group and perimenopause syndrome with mood disturbance group than in normal group（P0.05）,Which were 10.67±2.18,55.00±11.83 in mood disturbance group and 9.90±1.97,52.58±13.83 in perimenopause syndrome with mood disturbance group respectively.No significant difference was found in the two domain scores of quality of life between the perimenopause syndrome group and the normal group,there were significant difference between the perimenopause syndrome group and the perimenopause syndrome with mood disturbance group（12.79±2.38 vs 9.90±1.97）,（60.16±12.51 vs 52.58±13.83） respectively（P0.05）.The scores of mood symptoms,physical symptoms and sex in perimenopause were negative correlation with quality of life,especially the mood symptoms（P0.01）.Multiple stepwise regression analysis showed that depression and insomnia were the major determinants of the scores of quality of life.Conclusion Mood symptoms have more remarkable effects than the physical symptoms on social function and quality of life in perimenopause women.
