通腑法治疗重症肺炎（实热证）的临床观察

目的 观察通腑法治疗重症肺炎（实热证）的临床疗效及对重症肺炎预后的影响.方法 将71例患者随机分为两组,均予常规基础治疗,治疗组加用小承气汤鼻饲.比较两组氧合指数、血白细胞计数（WBC）、C反应蛋白（CRP）、临床肺部感染评分（CPIS）和28 d机械通气时间,28 d、60 d病死率.结果 经过2周的治疗,两组患者氧合指数均有改善,WBC、CRP、CPIS降低,两组比较,治疗组优于对照组（P＜0.05）;治疗组显效率高于对照组（P＜0.05）.结论 在常规治疗基础上加用通腑法可以降低重症肺炎患者CPIS评分,改善重症肺炎患者证候.

A Clinic Research of Tongfu Therapy on Patients with Excess-heat Syndrome of Severe Pneumonia

Objective：To investigate the clinical efficacy of Tongfu therapy on patients with excess-heat syndrome of severe pneumonia and its influence to the prognosis of severe pneumonia.Methods：Patients with excess-heat syndrome of severe pneumonia were divided into treatment group （35 cases） and control group （36 cases）.Conventional basic treatment was given to both groups.Additionally,small chengqi decoction was applied in treatment group for 2 weeks.Concentration of WBC,CRP,clinical pulmonary infection score （CPIS）,oxygenation index,duration of mechanical ventilation,and mortalities in 28 days and 60 days were detected to compare the clinical efficacy of the two groups.Results：After 2 weeks of treatment,oxygenation index gradually increased while concentration of WBC,CRP and CPIS in two groups gradually decreased.In treatment group,CPIS and concentration of WBC,CRP were lower than those of control group,and there were statistically significant differences （P＜ 0.05）.But there were no statistically significant differences of mortality rates in 28 days and 60 days respectively between control group and treatment group.Conclusion：On the basis of conventional western therapy,Tongfu therapy can not only decrease the CPIS of patients with severe pneumonia,but also improve their syndromes.
