摘    要:目的:探讨多囊卵巢综合征(PCOS)患者窦状卵泡卵母细胞结构变化。方法:对37例因不孕就诊且药物治疗无效的PCOS患者进行腹腔镜下多囊卵巢电凝穿刺或卵巢楔形切除术;共收集窦状卵泡卵母细胞121枚,在光学显微镜下进行观察。术后随访受孕情况至少半年,将患者分为妊娠组及未孕组,所得资料进行统计分析。结果:37例患者中有27例妊娠,观察其89枚卵母细胞,23例异常;未受孕10例中观察32枚卵母细胞,19例异常。术后PCOS未孕组卵母细胞异常的例数显著高于妊娠组(P<0.01)。结论:PCOS患者窦状卵泡自身卵母细胞结构异常导致卵泡未能发育成熟为优势卵泡,是顽固性PCOS患者不孕的主要因素之一。

Abstract:Objective: To investigate the structure change of oocytes of antral follicles in polycystic ovarian syndrome (PCOS) patients. Methods: 37 PCOS patients with infertility that unresponsive to prior-medicine therapy were treated by laparoscopic ovarian drilling (LOD) or ovary wedge resection. 121 oocytes of antral follicles were gotten during surgery and observed under the light microscope. And all the patients were followed-up at least half a year post-operation, the data was analyzed by statistics. Results: 27 of 37 women conceived after operation, and 23 of 89 oocytes in these 27 women were abnormal; 19 of 32 oocytes were abnormal in 10 women that fail to pregnant. There was more abnormal oocytes in non-conception group when compared to pregnant group after operation(P<0.01).Conclusions:The abnormality in oocyte itself cause that the antral follicle fail to develop dominant follicle is one of the main reasons for some PCOS women with refractory infertility.
