七氟醚联合瑞芬太尼快通道麻醉在甲状腺手术中的临床观察

目的观察七氟烷复合瑞芬太尼用于甲状腺手术的麻醉效果。方法选择行甲状腺手术的患者60例,随机分为七氟醚组和丙泊酚组,观察麻醉诱导前后、手术后30min及术毕的血压、心率变化及手术结束后患者清醒及拔管时间、苏醒期并发症的发生情况,并比较两组间瑞芬太尼的用量。结果两组患者在麻醉诱导前后及术毕的血压、心率变化差异无统计学意义。术毕停药后两组患者均迅速苏醒、拔管早,定向力恢复快。七氟醚组瑞芬太尼的用量明显低于丙泊酚组。结论甲状腺手术中应用七氟醚联合瑞芬太尼静吸复合麻醉,术中循环平稳,术毕苏醒迅速,并发症少,是值得临床推广应用的一种快通道麻醉方法。

Clinical observation on sevoflurane and remifentanil applied for the fast-track anesthesia in thyroidectomy

Objective To evaluate the anesthetic effect of sevoflurane combined with remifentanil in thyroidectomy. Methods 60 patients undergone thyroidectomy were randomly divided into sevoflurane group and propofol group,30 patients in each group. The changes of vital signs,anesthetic effect,interval before revival and untoward effects during the operation were observed. Results The anesthetic effect in these two groups was excellent. The changes in blood pressure and heart rate between these two groups showed no significant difference. In sevoflurane group,total dosage of remifentanil were significantly lower than those of propofol group. The difference in interval before revival between these two groups showed no statistical significance. Conclusion Sevoflurane combined with remifentanil showed excellent anesthetic effect. It can provide stable circulation during operation and make the interval before revival getting shorter. So it can be taken as a perfect anestheticmethod in thyroidectomy.
