老年人群肝酶异常的流行病学调查研究

目的 调查60岁以上老年人群丙氨酸氨基转移酶（ALT）异常患病率及其相关危险因素.方法 选择2008年体检的60岁以上的退休职工,对其进行血清肝酶水平流行病学的横断面研究,所有对象均排除肝炎病毒携带和有大量饮酒史者.结果 研究获得1 759例60岁以上的调查对象,其中92例（5.2%）被诊断为ALT异常.单因素Logistic回归分析显示：年龄、体重指数、腰围、甘油三酯与ALT异常相关（均P＜0.05）.而性别、血压、总胆固醇、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、空腹血糖与ALT指标异常无明显相关（均P＞0.05）.后对单因素回归分析中有统计学意义的变量进行多因素逐步Logjstjc回归分析,结果显示年龄、腰围与ALT异常相关（均P＜0.05）;年龄所致ALT异常的优势比（OR）为0.930,腰围所致ALT指标异常的OR值为1.075.结论 肥胖导致的老年人群肝酶异常需值得关注,生活和饮食习惯的改善对老年人群肝酶指标的改善非常重要.

A survey on abnormality of liver enzyme in elderly population

Objective To investigate the abnormality of liver enzyme in elderly population and related risk factors. Methods Blood samples were collected from 1759 retired workers aged over 60, who underwent health check in 2008, the HIV carriers and alcohol drinkers were excluded in the study. The serum alanine aminotransferase （ALT） levels, blood lipids and glucose levels were measured. Results Among 1759 subjects 92 （5.2%） were diagnosed as ALT abnormality. Univariate Logistic regression analysis demonstrated that age, body mass index, waist circumference and blood triglyceride were closely related to abnormal ALT level （（P 〈0.05）, while gender, blood pressure, total cholesterol, HDL cholesterol, LDL cholesterol and fasting plasma glucose were not related to abnormal ALT level （P 〉0.05）. Variable stepwise Logistic regression analysis showed that age and waist circumference were closely related to ALT abnormality. The odds ratio （OR） of age caused abnormal ALT was 0.930, while the OR for waist circumference was 1.075. Conclusions The problem of obese inducing liver enzyme abnormality should be concerned in elderly population, living habits and diet health should be concerned for elderly population to improve the level of liver enzyme.
