同期发生食管贲门双源癌16例诊治分析

目的探讨同期发生的食管贲门双源癌的外科治疗方法及疗效。方法 2002年8月—2010年7月,共收治同期发生的食管贲门双源癌16例,均为男性,年龄52～67(60.4±4.8)岁,均行手术治疗。采用左侧开胸切口行食管贲门双源癌切除术14例,胸腹联合切口行食管贲门双源癌切除2例,术中消化道重建用胃代食管胸腔内吻合术。结果本组围术期无死亡病例。术后发生阵发性心房纤颤2例,经治疗后缓解;肺部感染2例,给予抗感染治疗后治愈。术后切口感染2例,经换药痊愈。10例患者获得随访,1、3、5年生存率为100%、40%、20%。结论同期发生的食管贲门双源癌患者,经积极手术治疗,预后良好,生活质量明显提高。

Diagnosis and treatment of 16 cases of concurrent cancers from esophagus and gastric cardia

Objective To investigate the diagnosis and the treatment of concurrent cancers from esophagus and gastric cardia in the same patient.Methods A retrospective analysis was made through clinical data of 16 male patients(aged 52-67 years,mean = 60.4 ± 4.8) with concurrent cancers from esophagus and gastric cardia in the same patient from August 2002 to July 2010.Thoracotomy was performed in 14 patients,and thoracoabdominal incision was performed in the other two ones.Results No perioperative death occurred in the group.Two patients had post-operative paroxysmal atrial fibrillation,two patients had postoperative pneumonia,and two patients had postoperative incision infection.All these postoperative complications were cured.Ten patients were followed up and their 1-,3-and 5-year survival rate was 100%,40% and 20%,respectively.Conclusion Timely surgical treatment of concurrent cancers from esophagus and gastric cardia is safe and effective,and helps to improve the patients’prognosis and quality of life.
