纤维支气管镜引导经鼻气管插管抢救COPD重症呼吸衰竭病人的应用价值

目的:探讨纤维支气管镜引导经鼻气管插管在抢救COPD重症呼吸衰竭病人中的疗效。方法:50例COPD重症呼吸衰竭病人在纤维支气管镜引导经鼻气管插管后呼吸机辅助通气治疗,观察成功率、临床表现、血气分析及SaO2变化。结果:纤维支气管镜引导经鼻气管插管50例成功率100%,并发症少,操作时间短0.5～2min,血气分析pH、PaO2和PaCO2较前明显改善(P<0.01),SaO2上升。结论:纤维支气管镜引导经鼻气管插管抢救重症呼吸衰竭病人安全、迅速,插管成功率高,能有效纠正呼吸衰竭,改善通气,提高抢救病人的成功率。

Guided by Fiberbronchoscope Nasal Tracheal Intubation for COPD with Severe Respiratory Failure Patients

Objective:To investigate the fiberoptic bronchoscope guided nasotracheal intubation in rescuing patients with respiratory failure in the COPD effect.Methods:50 cases of COPD patients with severe respiratory failure in the fiber bronchoscope after nasotracheal intubation mechanical ventilation treatment,observe the success rate,clinical symptoms,blood gas analysis and SaO2 changes.Results:Guided by fiberbronchoscope through nasal tracheal intubation in 50 patients with a 100% success rate,fewer complications,short operating time 0.5~ 2 minutes,blood gas analysis of pH,PaO2 and PaCO2 significantly improved(P<0.01),SaO2 rose.Conclusion:Guided by fiberbronchoscope through nasal tracheal intubation in treatment of severe respiratory failure patient safety,rapid,the intubation success rate is high,can effectively correct the respiratory failure,improving ventilation,improve the success rate of emergency patients.
