不同自体植骨材料在不同植骨融合术腰椎滑脱患者中的对比分析

目的对比不同自体植骨材料在不同植骨融合术中对腰椎滑脱患者的临床效果。方法将河北北方学院附属第二医院治疗的97例腰椎滑脱患者分为自体棘突椎板横突间移植(A组)23例,自体棘突椎板椎体间移植(B组)27例,自体髂骨横突间移植(C组)24例和自体髂骨椎体间移植(D组)23例。对比4组患者治疗后神经功能改善情况和植骨后融合率。结果 A组神经功能改善总有效率为91.30%,B组为88.89%,C组为91.67%,D组为95.65%,4组间相比,差异无统计学意义(χ2=0.761,P>0.05)。随访6个月,4组融合率差异有统计学意义(χ2=9.749,P<0.05);其中A组与B、C组间、B组与C组间差异无统计学意义(P>0.05),D组融合率明显高于A、B、C 3组(P<0.05)。随访1年,4组间融合率差异无统计学意义(χ2=1.185,P>0.05)。结论选择自体髂骨作为植骨材料和椎体间植骨融合术的疗效较为明显,可作为临床治疗腰椎滑脱的方法之一。

Comparative analysis of different autogenous bone graft materials in patients with different bone graft fusion lumbar spondylolisthesis

Objective To compare clinical effect of different bone graft materials in patients with different bone graft fusion Lumbar spondylolisthesis.Methods 97 cases of lumbar spondylolisthesis were treated in The Second Hospital Affiliated to Hebei North College were divided into itransplantation autologous spinous process and Shii Yoko process（group A） 23 cases,the autologous spinous process laminectomy interbody graft（group B） 27 cases,autogenous iliac bone cross between transplantation（group C） 24 cases and autogenous iliac bone interbody graft（group D） 23 cases.the improvement of neurological function and bone graft fusion rate were compared among four groups after treatment.Results Total effective rate of group A was 91.30%,88.89% in group B,91.67% in group C,95.65% in group D,there was no statistically significant difference between the four groups（χ2=0.761,P0.05）.the fusion rate of four groups had significant difference after follow up for 6 months（χ2=9.749,P0.05）;group A and B,C,group B and C had no significant difference（P0.05）,group D was significantly higher than group A,B,C（P0.05）.The fusion rate showed no significant difference of four groups after followup for 1 years（χ2=1.185,P0.05）.Conclusion Curative effect might be obviously by using autologous hip bone as bone graft material and vertebrae fusion between the vertebral bodies,could be used as one method of clinical treatment of lumbar spondylolisthesis.
