莱芜地区出血性脑卒中患者的危险因素分析及干预对策

目的探讨莱芜地区出血性脑卒中患者的相关危险因素,并制定针对性的干预措施。方法选取2010年10月至2012年8月山东省莱芜市人民医院收治的出血性脑卒中患者183例(脑卒中组)以及同期健康体检者158例(对照组)。对两组人群临床资料、生活习惯等进行调查,并测量其身高、体质量,计算体质量指数(BMI),检验两组患者血脂水平和血浆纤维蛋白水平。结果单因素分析显示年龄大于70岁、BMI指数大于或等于25kg/m2、吸烟、有高血压病史、三酰甘油(TG)≥1.7mmol/L、总胆固醇(TC)≥5.7mmol/L、纤维蛋白原小于4.0g/L的患者脑卒中组明显多于对照组,两组比较差异有统计学意义(P<0.05)。多因素分析显示,年龄大于70岁、有高血压病史、BMI指数大于或等于25kg/m2、TC≥5.7mmol/L是老年患者出血性脑卒中的独立危险因素(P<0.05)。结论通过控制目标人群的血压和血脂,改变其不良生活习惯,合理膳食,适当锻炼,减少吸烟酗酒,调整心态,有利于预防出血性脑卒中。

The risk factors analysis and intervention measures of patients with hemorrhagic stroke in Laiwu area

Objective To explore the risk factors of patients with hemorrhagic stroke in Laiwu area for making corresponding intervention measures.Methods 183 cases of patients with hemorrhagic stroke（stroke group） and 158 cases healthy physical people（control group）were choosen in People′s Hospital of Laiwu City from October 2010 to August 2012.The clinical material and living habits were investigated,and the height,weight,BMI,inspection blood lipid level and plasma fiber protein level were measured among the two groups.Results Univariate analysis showed that age 70 years,BMI index ≥25 kg/m2,smoking,history of hypertension,triglyceride（TG） ≥1.7 mmol/L,total cholesterol（TC） ≥ 5.7 mmol/L,fibrinogen less than 4.0 g/L in patients with stroke was significantly more than the control group（P0.05）.Multivariate analysis showed that age 70 years,history of hypertension,BMI index ≥25 kg/m2,TC≥5.7 mmol/L were independent risk factors to patients with hemorrhagic stroke in elderly（P0.05）.Conclusion It would be advantageous to the prevention of hemorrhagic stroke by controling target population blood pressure and blood fat,changing the bad life habits,reasonable diet,proper exercise,reducing smoking drinking and adjusting state of mind.
