多联疗法治疗糖尿病周围神经病变短期疗效观察

目的 观察补阳还五汤加穴位注射弥可保治疗糖尿病周围神经病变(DPN)的临床疗效.方法 将60例患者随机分为3组各20例,Ⅰ组采用基础治疗加补阳还五汤治疗,Ⅱ组采用基础治疗加穴位注射弥可保治疗,Ⅲ组采用基础治疗加补阳还五汤加穴位注射弥可保治疗.观察3组疗效、临床症状、胫后运动神经传导速度、血流变学指标及血脂变化情况.结果 Ⅰ、Ⅱ、Ⅲ组总有效率分别为70.0%、65.0%、95.0%,Ⅲ组与Ⅰ、Ⅱ组比较有显著性差异(P＜0.05).Ⅰ组与Ⅱ组比较除口干症状外均无显著性差异,Ⅲ组与Ⅱ组比较诸症状均有显著性差异(P＜0.01).3组治疗前后运动神经远端潜伏期、传导速度、波幅均有改善,Ⅲ组与治疗前比较有显著性差异(P＜0.01);Ⅰ与Ⅱ组治疗后比较无显著性差异(P＞0.05),Ⅲ组与Ⅱ组治疗后比较有显著性差异(P＜0.05).Ⅲ组与Ⅰ组治疗前后比较全血黏度和血脂有显著性差异(P＜0.05),Ⅲ组与Ⅰ组治疗后比较无显著性差异,Ⅲ组与Ⅱ组治疗后比较有显著性差异(P＜0.05).结论 补阳还五汤加弥可保穴位注射治疗糖尿病周围神经病变疗效较好.

Observation of multi-therapy on diabetic peripheral neuropathy

Objective To observe the effect of Buyanghuanwu decoction combined with injectio ad acumen of Methycobal on the treatment of diabetic peripheral neuropathy.Methods 60 patients with diabetic peripheral neuropathy were randomly divided into three groups.Patients in Group one were treated by combination of Buyanghuanwu decoction and routine therapy. Patients in Group two were treated by combination of injectio ad acumen and routine therapy.Patients in Group three were treated by combination of Buyanghuanwu decoction,injectio ad acumen and routine therapy.Observe effect,symptoms,motor nerve conduction velocity,indexes of hemorrheology and change of blood fat.Results The effective rate in Group one,Group two and Group three were 70.0%,65.0%,95.0%,respectively.The effective rate in Group three was higher than that in Group one and Group two(P<0.05).Improvements of symptoms in Group three after treatment were superior to those in Group two(P<0.01).Motor nerve distal latency,conduction velocity and wave amplitude in three groups after treatment were obviously improved(P<0.01).There were significant differences between Group three and Group two in specific viscosity and blood fat(P<0.05).Conclusion Buyanghuanwu decoction combined with injectio ad acumen of Methycobal has a good effect on diabetic peripheral neuropathy.
