高压氧治疗家兔烧冻复合伤疗效观察

摘    要:目的 烧冻复合伤后在常规治疗基础上采用高压氧治疗,探索高压氧对提高烧冻复合伤的治疗效果。方法 用实验家兔在野外寒冷自然环境条件下作烧伤后再发冻伤动物模型,伤后经过清创处理并相应的常规治疗后,进行高压氧治疗,压力为0.25MPa,每日1次,共10次。结果 常规+高压氧治疗后,烧冻局部水肿于治疗1d后开始消退,3d后基本消退,5d后创面渗出消失。创面植皮3d开始生长,10d创面愈合,创面无感染。对照组局部水肿3d开始消退,5d后才有明显减轻。创面植皮5d后生长,2周后皮瓣成活75%。结论 烧冻复合伤在积极采用抗休克、止痛、补液、抗感染的基础上,加用高压氧治疗,疗效显著,并发症明显减少。

Abstract:Objective To explore the effect of hyperbaric oxygenation (HBO) on rabbits with burn-freezing combined injury,based on routine treatment.Methods In natural frigid zone,48 experimental rabbits were first made II degree burn and then II degree freezing injury to create animal models.After that, they were divided randomly into 3 groups:24 for no treatment;12 for routine treatment added with HBO ,0.25 MPa,once a day for 10 times together(HBO group);12 for mere routine treatment (control group). Results In the HBO group,1 day later, the local edema of burn-freezing wounds began to disappear and 3 days later disappeared completely; 5 days later ,the exudation also disappeared. Dermatoplasty on wound surface began to grow in 3 days,and healed after 10 days.No infection was found.In the control group,the burn-freezing wounds began to dissappear 3 days later and only had greater improvement 5 days later.Dermatoplasty grew 5 days later and the surviving rate of flap was only 75% after two weeks. Conclusion On the basis of routine treatment like anti-shock,relieving pain,supplementary body fluid and antiinfective, HBO treatment has a significant effect on burn-freezing combined injury and can decrease the complications greatly.
