慢性心力衰竭大鼠心肌肥厚指标的综合评价

目的进一步提高慢性心力衰竭心肌肥厚的诊治水平。方法将80只大鼠随机分为手术组和对照组。手术组腹主动脉缩窄法制备压力超负荷性慢性心力衰竭模型,对照组为假手术组。术后第4、8、12和16周时监测两组血流动力学指标,心功能指标,血清B型钠尿肽（BNP）、I型胶原水平,心脏重量指数及心肌细胞直径（CMD）。结果与对照组相比,手术组BNP水平从术后4周开始显著升高（P〈0.05）并在观察期间进行性升高;I型胶原浓度在术后12周达峰值,CMD均大于同时间段的对照组;术后8周左室内压下降最大速率（-dp/dt）降低,术后12周左室收缩末压（LVSP）升高,左室内压上升最大速率（＋dp/dt）降低;术后16周上述改变更明显。结论血清BNP、I型胶原水平及心肌细胞直径对于早期发现心肌肥厚最为敏感。

Comprehensive evaluation of different indexes of cardiac hypertrophyin Rats with chronic heart failure

Objective To further improve the diagnosis and treatment level of cardiac hypertrophy in patients with chronic heart failure. Methods Eighty Wistar rats were randomly divided into operation group and control group. The operation group was established chronic heart failure model by abdominal aortic banding mehods,the control group was sham-operated group. The hemodynamics indexes ,cardiac function index,serum brain natriuretic peptide（ BNP） and I type collagen level ,cardiac mass index and cardiac muscle cell diameter（ CMD） were investigated 4,8,12 and 16 weeks after operation group. Results Compared with the control group,BNP level of the operation group increased significantly since 4 weeks after operation（ P 0. 05） and increased gradually during the observation period ; I type collagen concentration reached peak at 12 weeks after operation,and CMD increased; the diastolic parameter of-dp/dt decreased at 8 weeks after operation,LV systolic pressure （ LVSP） increased and ＋ dp/dt decreased at 12 weeks of operation （ P 0. 05） ; the aboved changes became more significant at 16 weeks after operation. Conclusion Serum BNP,I type collagen concentration and CMD are very sensitive to find cardiac hypertrophy early.
