头面部血管畸形的血管内栓塞治疗及局部整形重建

摘    要:目的 探讨头面部血管畸形的栓塞治疗及局部整形重建特点。方法 回顾分析 11例头面部血管畸形临床资料。病例先后应用NBCA、丝线、颗粒或无水酒精等进行栓塞 ;4例栓塞后手术切除 ,其中 2例应用钛网进行局部重建。结果 栓塞后畸形团血管充盈减少 ;原有搏动及杂音减弱。手术切除彻底 ,无复发 ;局部重建者 ,外观轮廓完整。治疗后 8例痊愈 ,3例病变反复者经多次局部注射无水酒精 ,临床缓解症状良好。结论 栓塞治疗头面部血管畸形疗效良好 ,治疗时可根据病情和治疗阶段选用不同的栓塞材料和方法。病灶侵犯局部正常组织而影响外形者 ,要及时切除病变和局部重建

Abstract:Objective To elevate the embolization therapy and local reconstruction in vascular malformations on head and face.Methods 11 patients with extracranial arteriovenous malformation of the head and neck suffered different embolization were reviewed retrospectively.Four cases received operation in which 2 of them performed local reconstruction with shape memory NiTi alloy.Results After embolization both patients showed clinial and angiographic improvement.No recurrence occurred in 4 of the patients received single embolization therapy as well as in all of the patients received combined therapy.The out-like was satisfying in the cases suffered local reconstruction.Among the patients,8 cases cured and 3 cases kept control by pure alcohol injection directly.Conclusions Selective embolization is useful for vascular malformations of the head and face,and the selection depends on the manifestations and courses of the disease.Selective embolization,judicious surgical resection,composite reconstruction are the general therapy for invasive larger malformations.
