慢性肾衰竭患者同型半胱氨酸与心脑血管疾病的关系

摘    要:目的观察慢性肾衰竭(chronic renal failure,CRF)患者同型半胱氨酸(Homocysteine,Hcy)水平,分析同型半胱氨酸水平高低与心脑血管并发症的关系。方法选择CRF患者60例其中30例服用叶酸和维生素B6(VitB6)]和25例健康对照组,测定其血浆Hcy、肌酐、尿素氮、血糖、血脂、白蛋白。结果CRF患者血浆总Hcy水平明显高于健康对照组,差异有统计学意义(P〈0.001),其中并发心脑血管疾病组总Hcy水平明显高于未并发组,差异有统计学意义(P〈0.05);服用叶酸、VitB612周前后Hcy水平分别为(22.72±15.20)μmmol/L,(11.86±7.15)μmmol/L(P〈0.05)。结论慢性肾衰竭患者普遍存在高同型半胱氨酸血症,并可能参与了心脑血管疾病的发生,服用叶酸、VitB6可以减低Hcy水平,从而减少心脑血管疾病的发生。

Abstract:Objective To investigate the relationship between homocysteine(Hcy) level and cardiovascular events in patients with chronic renal failure(CRF). Methods 60 patients with CRF( in which 30 cases received folic acid and VitB6 treatment I and 25 healthy controls were enrolled in the study. The levels of serum homocysteine, creatinine, blood urea nitrogen, blood glu- cose, triglyceride( TG), cholesterol( CHO), albumin(Alb) and so on were measured. Results The serum level of Hcy m patients with CRF was obviously higher than that in the control group(P 〈 0.001 ). and serum level of Hcy was higher in cases with cardio-cerebrovascular diseases than that in cases without ( P 〈 0.05). The Hcy level were ( 22. 72 ± 15. 20)μmmol/L and ( 11.86 ± 7.15 ) μmmol/L after 12-week treatment with folic acid and V.itB6. Conclusions The high level of Hcy is universal in patients with CRF and may contribute to the cardiovascular disease. The treatment with folic acid and VitB6 can decrease the levels of Hcy and reduce the risks of cardiovascular diseases.
