高强度超声治疗兔肺部早期肿瘤

摘    要:目的:探讨高强度超声(high intensity ultrasound,HIU)治疗兔肺部VX2早期肿瘤的有效性和安全性。方法:将建立肺癌模型4d的32只实验兔随机分为三组,分别为手术组、HIU治疗组及假照组。HIU治疗组于术后7d随意处死2只取材作病理切片和电镜检查,观察局部及肺门淋结变化,另10只与手术组及对照组进行比较,监测各组肺部功能前后变化情况,平均生存时间,生存质量及治愈情况,结果:病理报告示肿瘤治疗区域均凝固性坏死,周围肺组织充血,肺门淋巴结未见肿瘤细胞浸润;电镜示肿瘤细胞膜被破坏,细胞核内染色质边集,分散,手术组与HIU治疗组术前,术后肺部功能无显著差异,平均生存时间均明显高于假照组,两组生存时间与治愈率无差别。结论:高强度超声治疗兔肺部VX2早期肿瘤是安全有效的,有效成为肺部肺肿瘤微创外科的又一治疗手段。

Abstract:Objective To study on the efficacy and security of high intensity ultrasound(HIU) in the treatment of early stage rabbit lung VX 2 tumor.Methods Thirty-two rabbits(with lung tumors for 4 days)were randomly divided into three groups,group A( n =12)received 200 seconds HIU sonication with frequency of 5.5MHz,Ultrasound beam was set to extend 1 cm around the tumor;group B( n =10)received surgery therapy;group C( n =10)was control group.In group A,2 of 12 were killed on the 7th day after ultrasound exposure.We observed pathological changes of targeted tumor with microscopy.The other 10 rabbits were still alive for more than 150 days.The survival time of each rabbit and the cure rate of each group were recorded.Lung functions were measured before and after surgical and HIU operations;also we observed the lymph nodes changes.Results Pathological and electrical microscope findings showed that tumor cells were dead irreversibly.Compared with the control group,the groups that received therapy survived significantly longer( P <0.05).There were no significant differences in tumor cure rates and survival time between the group receiving operation therapy and the group receiving HIU treatment.The cure rate is zero in group C,100% in group A and B.Conclusions HIU can treat early stage lung cancer safely and effectively.It will be one of the modalities in lung cancer treatment.
