经腹直视下胃冠状静脉栓塞术治疗门静脉高压症

用经腹直视下胃冠状静脉栓塞加脾切除的断流术治疗５０例门脉高压症合并食道胃底曲张静脉破裂出血患者，结果急性大出血病人术后均能立即止血．住院死亡率仍有８％，与术前肝功严重损害及操作不当有关．随访结果显示远期效果较好．虽有８．６％再出血率，但与肝功及栓塞范围不够有关．该术式需时短，操作简单，创伤小，病人易耐受，断流较完全，但有异位栓塞的可能．只要严格按照操作规程和要求，异位栓塞可明显降低．若术前能尽量改善肝功能，疗效将进一步提高．

Treating Portal Hypertension by Gastric Coronary Vein Embolism with TH-glue Under Transperitoneal Macroscopy

Transperitoneal gastric coronary vein embolism plus splenectomy in direct vision was performed on 50 patients.It is characterized by simplicity in procedure,a high success rate of immediate hemostasis,a complete devascularization and a good remote result.The incidence of ectopic embolism may be decreased if the operation specifications and rules are strictly followed.The transperitoneal gastric coronary vein embolism is suggested to be an effctive approach to treat portal hypertension with esophagogastric varicose hemorrhage.The point of procedure,the causes and preventive measures of ectopic embolism are introduced in this report,too.
