经胭静脉穿刺介入治疗髂股静脉血栓形成

目的探讨经胭静脉穿刺介入治疗髂股静脉血栓形成的临床疗效。方法对32例髂股静脉血栓形成患者经胴静脉穿刺行综合性介入治疗，经健侧股静脉置入下腔静脉滤器（永久性或临时性滤器），然后经患侧胭静脉入路，行经导管接触性溶栓术、血栓清除术、经皮腔内球囊或（和）支架成形术综合治疗。结果胴静脉一次穿刺成功率为100％，滤器置入均取得成功，32例患者综合介入治疗后有17例（53．1％）患者血栓完全清除，血流恢复，7例（21．9％）血栓大部分清除，5例（15．6％）血栓部分清除，3例（9．4％）无效。患者随访2-12个月，1例血栓部分清除患者随访期内症状复发。结论经胭静脉穿刺介入治疗髂股静脉血栓形成是一种安全、有效、微创的治疗手段。

Interventional treatment of iliofemoral venous thrombosis through popliteal vein puncture

Objective To evaluate the clinical value of popliteal vein puncture in the interventional treatment of iliofemoral venous thrombosis(IFVT).Methods Thrity-two cases with iliac strands of venous thrombosis after the popliteal venipunc ture line comprehensive intervention,the lateral femoral venous health into the inferior vena cava filter(permanent or tem porary filter),and then the popliteal veins of the access,through sexual contact,embolus removal surgery,the percutenous transcathater balloon angioplasty(PTBA) or(and) stent were performed through popliteal vein access in the treatment pro cedure.Results The successful rate of the first popliteal vein puncture was 100%,and all of the IVC filters were placed successfully.Iliofemoral venous thrombolysis had complete clot lyses or removed with recanalization of vascular in 17 cases(53.1%).7 cases(21.9%) had complete clot lyses,and 5 cases(15.6%) had partial clot lyses,3 cases(9.4%) had no clot lyses.1 case of partly clot lyses recurred during 2-12 months follow-up.Conclusion Interventional treatment of iliofemoral venous thrombosis is a safe,effective and micro-invasive method.
