缺血性脑卒中患者脑动脉狭窄的好发部位及危险因素的研究

目的探讨广州地区缺血性卒中患者脑动脉狭窄好发部位,分析其与危险因素的相关性。方法对355例广州籍缺血性脑卒中患者利用Essen卒中风险评分量表（ESRS）评分分组。同时按照脑血管造影（DSA）结果分组。记录年龄、家族史、吸烟、饮酒等影响因素。结果患者脑血管造影显示：无血管狭窄70例,不同程度血管狭窄285例。青年以颅内动脉狭窄为主,中年以颅内血管狭窄及合并颅内外血管狭窄为主,老年以颅外血管狭窄为主。颅内外动脉狭窄常见部位为：大脑中动脉163条,椎动脉156条。高血压、周围血管病变为脑血管狭窄的独立危险因素。结论不同年龄组动脉狭窄部位分布有差异,ESRS评分能有效预测脑动脉狭窄风险。

Distribution and risk factors of cerebral artery stenosis in patients with ischemic stroke

Objective To investigate the distribution of cerebral artery stenosis in patients with ischemic stroke,to analyze its related risk factors.Methods 355 cases of ischemic stroke in Guangzhou nationality patients were divided into three groups according to ESRS score and digital subtraction angiography(DSA)result.To record the age,family history,smoking,alcohol consumption and other factors.Results Cerebral angiography showed:70 patients without stenosis,varying degrees of vascular stenosis in 285 cases.Young people mainly had intracranial arterial stenosis,middle-aged man had intracranial arterial stenosis,or both intracranial arterial stenosis and extracranial arterial stenosis,the elderly mainly had extracranial arterial stenosis.Common site of intracranial arterial stenosis were:163 branchs of middle cerebral artery,156 branchs of vertebral artery.Hypertension,peripheral vascular disease for the cerebral vascular stenosis were independent risk factors.Conclusion Artery stenosis in different age groups are different,ESRS score can predict the risks cerebral artery stenosis.
