危重新生儿凝血指标与危重评分的临床分析

目的探讨危重新生儿凝血功能指标与危重评分的关系。方法对2008年11月～2011年11月入住本院的137例新生儿进行危重评分,并检测凝血指标,比较患儿凝血指标与疾病严重程度的关系。结果危重足月儿D-二聚体（D-D）各组间相比差异有统计学意义（P均〈0.05）,病情越重,D-D水平越高;危重早产儿凝血酶原时间（PT）、部分活化凝血活酶时间（APTT）及D-D各组间相比差异有统计学意义（均P〈0.05）,随病情加重,PT、APTT延长,D-D水平增高;相同危重程度各组早产儿D-D水平与足月儿相比,差异有统计学意义（P〈0.05）。结论 D-D与新生儿的胎龄及疾病的严重程度相关,危重早产儿更易发生凝血功能障碍。

Relationship between coagulation function and critical score in the neonates

Objective To investigate the relationship of coagulation function and critical score in the neonates with asphyxia. Methods The neonatal critical illness score (NCIS) were collected in 137 neonates, and their coagulation function were measured at same time. Data comparation and analysis were done. Results D-Dimer of each group was significantly different (all P < 0.05), there was positive correlation between D-Dimer and NCIS in full-term infants, with the infants’ critical condition getting worse, D-Dimer level increased. There were positive correlation between prothrombin time (PT), activated partial thromboplastin time (APTT), D-Dimer and NCIS inpreterm infants. With the infants’ critical condition getting worse, PT and APTT prolonged,D-Dimer level increased, the PT, APTT, D-D of each group was significantly different (all P < 0.05). In same NCIS level condition, preterm infant had a higher D-Dimer level than term infant (P < 0.05). Conclusion D-Dimer levels may reflect the severity of disease in newborn infant. Higher D-Dimer level in preterm infants shows that they are more vulnerable to coagualtion dieturbance.
