乙型肝炎肝硬化与糖代谢异常的临床研究

目的探讨检测乙型肝炎肝硬化患者糖代谢的异常变化。方法检测对比乙型肝炎肝硬化患者（A组）、2型糖尿病患者（B组）以及正常健康者（C组）的空腹血糖（FPG）、2h血糖（2hPG）、糖化血红蛋白（HbA1c）、胰岛素、C肽浓度以及胰岛素释放指数（IRI）。结果 （1）A组FPG、2hPG、HbA1c、胰岛素、C肽浓度以及IRI均明显高于C组（P〈0.05）。（2）A组与B组多种指标比较,FPG水平低于B组（P〈0.05）;2hPG、HbA1c低于B组（P〉0.05）;而胰岛素、C肽浓度以及IRI明显高于B组（P〈0.05）。结论乙型肝炎肝硬化患者糖代谢异常可能是由于高胰岛素血症和胰岛素抵抗共同作用的结果。

The clinical research of hepatitis B liver cirrhosis and sugar metabolism disorders

Objective To explore the abnormal change of test the sugar metabolism anomalous of hepatitis B patients with cirrhosis. Methods The FPG, 2hPG, HbA1c, insulin, c-peptide concentration and IRI were compared in hepatitis B patients with cirrhosis(A group), type 2 diabetes patients (B group) and normal healthy person (C group). Results (1) The FPG, 2hPG, HbA1c, insulin, c-peptide concentration and IRI of A group were significantly higher than that in C group (P < 0.05). (2) Various index were compared between A group and B group, the FPG levels in A group was below than that in B group (P < 0.05), 2hPG and HbA1c were below than that in B group (P > 0.05); But the insulin, c-peptide concentration and IRI were obviously higher than that in B group (P < 0.05). Conclusion Hepatitis B patients with cirrhosis sugar metabolism disorders may be due to high insulin hematic disease and insulin resistance together.
