益肾胶囊对糖尿病肾病大鼠足细胞损伤的影响

摘    要:目的:观察益肾胶囊对糖尿病肾病大鼠足细胞损伤的影响。方法:32只大鼠随机分为正常对照组、糖尿病肾病模型组(模型组)、益肾胶囊组、苯那普利组,每组各8只。益肾胶囊组每日每只灌胃益肾胶囊(2.5g·kg^-1·d^-1),苯那普利组每日每只灌胃苯那普利(2.5g·kg^-1·d^-1),正常对照组及模型组每日给予等量的蒸镏水。测定血压、24h尿蛋白定量、血肌酐(Scr)、尿素氮(BUN),并计算内生肌酐清除率(Ccr);光镜检查肾组织病理变化,电镜及免疫荧光法检测肾组织及尿液中足细胞变化等。结果:实验24周后,模型组大鼠血压、24h尿蛋白定量、BUN、Scr较正常对照组明显增高(均P〈0.05),益肾胶囊组和苯那普利组可降低糖尿病肾病大鼠血压、24h尿蛋白定量、BUN、Scr(均P〈0.05)等;模型组足细胞podocalyxin蛋白表达明显低于正常对照组(P〈0.05),益肾胶囊组及苯那普利组大鼠足细胞podocalyxin蛋白表达较模型组显著增加(P〈0.05);两治疗组之间比较无统计学差异。正常对照组尿液偶见podocalyxin阳性足细胞,模型组尿液足细胞排泄较正常对照组明显增多(P〈0.05),益肾胶囊组及苯那普利组,大鼠尿液足细胞排泄减少(P〈0.05)。结论:益肾胶囊可通过减轻糖尿病肾病大鼠足细胞损伤而具有一定的肾保护作用。

Abstract:Objective:To observe the effect of Yi shen jiao nang on the podocytes injuries of rats with diabetic nephropathy.Methods:32 rats were randomly grouped into four groups, as control, model,Yi shen jiao nang treatment and Benarapril treatment;8 rats each group.Two days after models’ establishment, the rats were treated with yi shen jiao nang(2.5 g/kg)/Benarapril(2.5 g/kg), ig one a day, the control or model group was treated with the same amount of water. Blood pressure, urinary protein/24 h, serum creatinine(Scr), blood urea nitrogen(BUN), and endogenous creatinine clearance rate(Ccr) were measured,and optics microscope was adopted to observe the renal pathological changes.The podocytes in renal tissue and urine were investigated by electronic microscope and immunofluorescence respectively.Results:After 24 w, average arterial pressure, urinary protein/24 h, Scr and BUN in model group were significantly higher compared with control group(respectively,P〈0.05). After treatment with Yi shen jiao nang/ Benarapril, the average arterial pressure, urinary protein/24 h, Scr, BUN were reduced (respectively, P〈0.05).In control group, there was high level expression of podocalyxin.In the model group,expression of podocalyxin was significantly reduced (respectively,P〈0.05).After treatment of Yi shen jiao nang/ Benarapril, expression of podocalyxin was obviously increased (respectively,P〈0.05).There were few podocalyxin positive podocytes in urine of control rats, and podocytes were significantly increased in urine of model rats, compared with control rats. After treatment of Yi shen jiao nang/ Benarapril, podocytes were decreased in urine.Conclusion:Yi shen jiao nang protected diabetic nephropathy rat models from kidney damage probably by reducing the podocytes injuries.
