五酯片对口服他克莫司肾移植受者的影响

目的：探讨肾移植受者术后联用五酯片的安全性、有效性和经济性。方法：自2007年6月～至2009年6月该中心施行同种异体尸肾移植并采用FK506＋吗替麦考酚酯（MMF）＋醋酸泼尼松（Pred）为免疫抑制方案的患者中,随机选取病例纳入治疗组,术后以FK506联合五酯片口服,另随机选取免疫抑制方案相同患者作为对照。随访期满1年后比较两组间FK506用量、移植肾功能、肝功能（ALT）、急性排斥反应（AR）发生率、肺部感染发生率。结果：两组AR及肺部感染发病率比较差异无统计学意义。两组患者每日FK506用量自服用五酯片1月后开始即差异有统计学意义,治疗组服用FK506剂量比对照组低。但两组术后1月、3月、6月及1年复查FK506血药浓度、移植肾功能、肝功能损害发病率差异均无统计学意义。结论：肾移植术后1年以内,五酯片的应用在FK506用量大幅减少的情况下使治疗组取得了与对照组相同的移植效果,肾移植术后联用五酯片有望成为安全、有效的途径来减少社会和患者的经济负担。

Impact Upon Kidney Transplant Objects by Orally Taking Tarcolimus for a Continuous Taking of Chinese Magnoliavine Tablets

Objective：To explore the safety,effectiveness and economization of a long-term taking of Chinese Magnoliavine Tablets by a patient after receiving a kidney transplant operation.Methods：From June 2007 to June 2009,this center implemented kidney transplant upon different objects of the same type by an immune restriction plan of FK506＋MMF＋Pred.The patient cases were chosen in random as objects for treatment.After the operation was implemented,FK506 and Chinese Magnoliavine Tablets were both taken while the same objects of the immune restriction plan were taken for comparison.One year later,two teams were compared for the taken quantity of FK506,function of the transplanted kidney,ALT,AR occurrence probability,and lung infection probability.Results：There is no obvious statistical difference for the two teams in respect to AR and lung infection.The two teams of patients had obvious difference after taking the Chinese Magnoliavine Tablets for 1 year,and the treated team members took FK506 for a less dosage compared to that of the reference team.The two teams of candidates were re-tested for their blood medicine concentration of FK506,function of the transplanted kidney,disease incidence of liver due to damage respectively at 1 month,3 months,6 months and 1 year after the operation was implemented,and there was no statistical significance achieved.Conclusion：The treatment group obtained the same curative effect as that of the control group with notably reduced dosage of FK506 for 1 year after the renal transplantation.So it is hopeful for taking FK506 with Chinese Magnoliavine Tablets to reduce the economic burden of the society and the patients as a safe and effective method.
