摘    要:目的:探讨经胸乳径路腔镜甲状腺手术的临床效果。方法:全麻下行胸乳径路腔镜甲状腺切除术91例,其中结节性甲状腺肿48例,甲状腺腺瘤38例,甲状腺乳头状癌5例。结果:91例均顺利完成手术,手术时间50~195min,平均(120.5±29.6)min;术后发生颈部皮下积液和一过性声音嘶哑各1例。随访2~44个月,中位数21个月,无复发,患者对手术及美容效果均表满意。结论:胸乳径路腔镜甲状腺切除术是安全可行的,具有很好的美容效果。

Abstract:Objective To study the therapeutic effect of endoscopic thyroidectomy via chest-breast approach.Methods Endoscopic thyroidectomy was performed on 91 patients via chest-breast approach.Among the patients,48 had nodular goiter,38 had thyroid adenoma,5 had thyroid carcinoma.Results The endoscopic thyroiectomy succeeded in all patients.The operation time was 50~195 min(mean,(120.5±29.6) min),the blood loss was 5~30 mL(mean,(15.3±7.4) mL),hospitalized days were 5~8 d(mean,7.1 d).Complications occurred in 2 cases,including 1 case of accumulating liquid of subcutaneous in the neck,and 1 case of temporary hoarseness.No injury of parathyroid glands or the trachea was found.All 91 patients were followed-up for 2 to 44 months(mean,21 months).None of them had recurrence.Conclusion Endoscopic thyroidectomy via chest-breast approach is a Convenient and effective method with good cosmetic results.
