经胸乳径路腔镜甲状腺切除术91例

目的：探讨经胸乳径路腔镜甲状腺手术的临床效果。方法：全麻下行胸乳径路腔镜甲状腺切除术91例,其中结节性甲状腺肿48例,甲状腺腺瘤38例,甲状腺乳头状癌5例。结果：91例均顺利完成手术,手术时间50～195min,平均（120.5±29.6）min;术后发生颈部皮下积液和一过性声音嘶哑各1例。随访2～44个月,中位数21个月,无复发,患者对手术及美容效果均表满意。结论：胸乳径路腔镜甲状腺切除术是安全可行的,具有很好的美容效果。

Treatment of Thyroid Diseases with Endoscopic Thyroidectomy via Anterior Chest Wall Approach:Report of 91 Cases

Objective To study the therapeutic effect of endoscopic thyroidectomy via chest-breast approach.Methods Endoscopic thyroidectomy was performed on 91 patients via chest-breast approach.Among the patients,48 had nodular goiter,38 had thyroid adenoma,5 had thyroid carcinoma.Results The endoscopic thyroiectomy succeeded in all patients.The operation time was 50～195 min（mean,（120.5±29.6） min）,the blood loss was 5～30 mL（mean,（15.3±7.4） mL）,hospitalized days were 5～8 d（mean,7.1 d）.Complications occurred in 2 cases,including 1 case of accumulating liquid of subcutaneous in the neck,and 1 case of temporary hoarseness.No injury of parathyroid glands or the trachea was found.All 91 patients were followed-up for 2 to 44 months（mean,21 months）.None of them had recurrence.Conclusion Endoscopic thyroidectomy via chest-breast approach is a Convenient and effective method with good cosmetic results.
