儿童期口腔颌面部血管瘤综合治疗

【目的】探讨儿童期口腔颌面部血管瘤的有效治疗手段。【方法】320例儿童期口腔颌面部血管瘤,其中婴儿期220例,幼儿期40例,学龄前期26例和学龄期34例,用包括类固醇类药物口服、硬化剂注射、液氮冷冻、激光和放射性核素敷贴等保守方法及整形外科手术切除等综合治疗。【结果】治疗后半年至4年随访,保守治疗组中,类固醇、硬化、激光、冷冻和核素,治疗治愈率分别为23.6%,22.2%,28.6%,25.0%和26.1%,整形外科手术组治愈率为77.8%,外科治疗组与保守治疗组间统计学上差异有显著性(P<0.01)。【结论】保守治疗能治愈小部分病例,大部分治疗有效,可作为综合治疗的第一步。整形外科手术切除是治疗口腔颌面部血管瘤的理想和有效手段。

Comprehensive Treatment of Oral and Maxillofacial Hemangiomas in Children

To explore an effective treatment method for oral and maxillofacial hemangiomas (OMH) in children.320 patients with OMH including 220 newborns,40 infants, 26 cases at preschool age and 34 cases at school age , were treated comprehesively by conservative methods including corticosteroid drugs, sclerosing, frozen, laser, radionuclide and plastic surgical operation. Followed up by 6 months to 4 years after treatment, the cure rate in corticosteroid, sclerosing, frozen, laser and radionclide groups was 23.6%,22.2%,28.6%,25.0%and 26.1%respectively and was 77.8%in plastic surgical group.Statistical difference was found between the surgical group and the conservative group (P< 0.01).Conclusion]A small proportion of patient with OMH could be cured by conservative methods,which were effective for most OMH patients. Conservative methods can be used as the first step of comprehensive therapy. Pastic surgical resection is a feasible and curative method for OMH.
