平衡检测系统在康复医学中的应用

摘    要:平衡功能障碍的评定与治疗是康复医学的重要课题之一。19世纪中叶,Romberg首先应用行为检查方法对平衡障碍进行定性观察。70年代初,采用定量分析法,利用力台记录人在站立(静态平衡)时身体摆动的轨迹和范围。90年代以来,随电子计算机技术的发展,采用人体计算机模型准确地测量静、动态平衡功能状况。 平衡检测系统(balance master system,BM)主要由力合、可移动包围箱、监视器、计算机及专用软件构成。BM系统是静、动态站立平衡功能的评定与训练工具。其独特的动态性能使之还可以对平衡控制的感觉传输功能进行评定与训练。BM系统通过系统的控制和分离各种感觉信息输入评定躯体感觉、视觉、前庭系统对于平衡及姿势控制的作用与影响。在平衡的训练中,通过支持面和视环境的运动刺激感觉系统功能的恢复。当感觉损伤不可逆转时则促使正常的感觉系统进行代偿,最终达到平衡控制的能力。 BM系统可用于许多疾病引起的平衡障碍的评定与训练。BM系统的技术代表了当今平衡障碍评定与康复的发展方向。具有更广阔的临床应用前景。

Abstract:Assessment and training of balance control is animportant field in rehabilitation medicine. In the middle ofthe 19th century, the performance tests were first developedby Romberg. In the beginning of 1970s, forceplate began tobe used in recording and measuring the locomotion of thecenter of the vertical forces while the patient standing on theforceplate surface. With the advancement of electronicalcomputer technology in 1990s, a computer model of bodydynamics is used to derive the center of gravity sway from thecenter of vertical force movements.The balance master (BM ) System consists of moveable for-ceplate, moveable visual surround, monitors, 486DX IBMcompatible computer and system software. BM System offersquantitative assessment and training of static and dynamicstanding balance. Control of movement is measured throughthe limits of stability (LOS) at different speeds and direc-tions. The special dynamic features of the system permittesting and training the function of sensory inputs to balancecontrol. During sensory testing, the movement of the supportsurface and/or visual surround are" sway - referenced" tothe patient's postural sway. This unique, patented protocolsystematically alters the accuracy of available somatosensoryand visual cues to assess the use of somatosensory, visual,and vestibular inputs. During balance training, movement ofthe surface and/or surround may be independently set topermit individually tailored exercise programs. Impairedsensory systems can be stimulated or, when impairments arepermanent, the compensatory use of intact systems can bepromoted.The BM system is applicable to a broad range of patients.
