阴道超声下卵巢呈多囊性改变52例临床观察

目的分析在阴道超声下表现为多囊卵巢患者的临床特点。方法对我院妇科52例在阴道超声下卵巢呈多囊性改变患者的临床资料进行分析,并根据是否合并长期无排卵或雄激素升高的临床和(或)生化依据分为多囊卵巢综合征(PCOS)组及非PCOS组,比较两组间卵巢形态学特征及代谢变化。结果52例患者中PCOS组33例,占63.5%;非PCOS组19例,占36.5%,其中高催乳素血症(HPRL)3例(5.8%),甲状腺功能减退2例(3.8%),单纯性肥胖3例(5.8%),卵巢肿瘤2例(3.8%),妊娠2例(3.8%),特发性血小板减少1例(1.9%),正常妇女6例(11.5%),其中4例3个月内曾服用过口服避孕药。PCOS组的双侧卵巢体积、卵泡数及体质指数(BMI)均大于非PCOS组,但两者相比差异无显著性;而PCOS组睾酮水平、黄体生成素/卵泡刺激素均明显高于非PCOS组,差异有显著性。结论引起卵巢呈多囊性改变的原因有多种,其中PCOS为主要原因,卵巢多囊性改变是机体多种功能性紊乱的最终结果,而不是中枢或局部的特异性疾病。

Clinical Analysis of 52 cases with polycystic ovary in transvaginal ultrasound

Objective To analysis the clinical characteristics of patients with the performance as polycystic ovary in transvaginal ultrasound.Methods In the present study,fifty two patients with the performance as polycystic ovary in transvaginal ultrasound were selected from women who visited the gynecology endocrine out-patient department in our hospital.Their clinical data were analysised and were divided into polycystic ovarian syndrome(PCOS) group and non-PCOS group according to whether performing long-term anovulation and(or) hyperandrogenism.To compare the ovarian morphologic changes and metabolic features between two groups.Results Among total fifty two patients,there are 33 cases of PCOS group,accounts for 63.5%;19 cases of non-PCOS group,accounts for 36.5%.The bilateral ovarian volume,the number of follicles and BMI of PCOS group were larger than that of non-PCOS group,but no significant difference.The level of testosterone,LH/FSH of PCOS group were significantly higher than those of non-PCOS group.Conclusions There are many causes due to the change of polycystic ovary.The main cause is of PCOS.Polycystic ovary changes are the ultimate results of a variety of functional disorders,rather than central or local specific diseases.
