中药内服联合保留灌肠治疗慢性肾脏病系统评价

目的:利用Meta分析系统评估中药内服联合保留灌肠治疗慢性肾脏病的临床治疗效果及不良反应发生率。方法:利用计算机检索国内外主要电子数据库,辅助其他检索方式,按纳入标准纳入研究并提取资料及质量评价,使用RevMan5.3软件对数据进行Meta分析。结果:最终纳入22篇临床RCT文献,中药内服联合保留灌肠治疗慢性肾脏病的临床总有效率优于单纯西医治疗对照组(RR=1.33,P<0.00001,95%CI:1.26-1.41)。与对照组相比,治疗组降低血肌酐、尿素氮,升高肾小球滤过率、血红蛋白,消减蛋白尿等方面更为显著(P<0.00001)。结论:中药内服联合保留灌肠治疗慢性肾脏病在临床总有效率、延缓肾脏衰竭等方面具有一定的优势。另一方面,由于受所纳入文献质量及临床实施偏倚的影响,此结论还需要更多高质量、大样本同时设计严谨的科学试验进一步加以验证。

Systematic Evaluation of Chronic Kidney Disease Treated by Oral Chinese Medicine Combined with Retention Enema

Objective:To use Meta analysis system to evaluate the clinical therapeutic effect and the incidence of adverse reactions of traditional Chinese medicine combined with retention enema in the treatment of chronic kidney disease.Methods:Computers were used to search major domestic and foreign electronic databases,assisted by other search methods,included in the study according to the inclusion criteria,and extracted data and quality evaluation,and used RevMan5.3 software to conduct meta-analysis on the data.Results:Finally,22 clinical RCT literatures were included.The total clinical effective rate of oral Chinese medicine combined with retention enema in the treatment of chronic kidney disease was better than that of the control group(RR=1.33,P<0.00001,95%CI:1.26-1.41).Compared with the control group,the treatment group decreased blood creatinine and urea nitrogen,increased glomerular filtration rate,hemoglobin,and reduced proteinuria more significantly(P<0.00001).Conclusion:Oral Chinese medicine combined with retention enema in the treatment of chronic kidney disease has certain advantages in terms of clinical total effective rate and delay of renal failure.On the other hand,due to the quality of the included literature and the bias of clinical implementation,this conclusion needs to be further verified by more high-quality,large-sample and rigorously designed scientific trials.
