1078例胰岛素瘤的定位诊断

总结1981年1月至1999年10月国内文献报告的胰岛素瘤的定位诊断方法及结果。方法：应用中国医学科学院医学信息研究所“中国生物医学文献数据库”，光盘（1980年1月～1999年10月），以“胰腺内分泌肿瘤”及“胰岛素瘤”为主题词检索，去除无关文献、重复文献及无定位诊断方法的文献等，共得到92篇胰岛素瘤文献进行总结。结果：国内文献报告胰岛素瘤的定位诊断方法12种，分三大类：术前非侵入性、术前侵入性及术中定位方法。对这12种方法进行介绍并对其阳性率进行总结。结论：胰岛素瘤的各种定位检查方法的选用应根据病人情况及本单位的条件经验而定。各种侵入性方法并不推荐常规应用，仅适用于术中探查阴性病人、复发再次手术者或疑为多发性胰岛素瘤的病例。

Localization ofInsulinoma: Analysis of 1078 Cases.

To assess the role of pre- and intra-operative diagnostic procedures for the precise localization of insulinoma. Methods: All the data were selected from the Chinese Bio-Medical Data- base. The feasibility and accuracy of different diagnostic tools for the localization of insulinoma were reviewed. Results: Over a 19-year period (Jan 1981 to Oct 1999), 1078 patients underwent 12 different diagnostic procedures to localize the insulinoma, which were cataloged into three groups: preoperative non-invasive procedures; preoperative invasive procedures and intra-operative proce- dures. The accuracy is varied from one procedure to another. Conclusions: The invasive diagnostic imaging procedures don't need to be routinely carried out to localize the insulinoma, except the re- current cases, multiple lesions and negative findings during the operation via the traditional tech- niques.
