E-cadherin和MMP7在胆囊癌组织中的表达及其临床意义

目的检测E-cadherin和MMP7在胆囊癌组织中的表达情况，探讨其与局部浸润、远处转移及预后的关系以及两者的相关性。方法用免疫组化SP法检测63例胆囊癌中E-cadherin及MMP7的表达，10例慢性胆囊炎作为对照。结果癌组织中MMP7阳性率明显增高，其阳性表达与胆囊癌的TNM分期、肝浸润及淋巴结转移有关（P〈0．05）；癌组织中E-cadherin阳性表达率下降，阳性信号紊乱，呈点状、短线状，其表达降低与肿瘤分期、淋巴结转移及神经浸润有关（P〈0．05）。Speaman相关分析表明，MMP7与Bcadk曲在胆囊癌组织中的表达呈负相关（P〈0．05）。生存分析提示MMP7阳性表达、肿瘤TNM分期及神经浸润与术后生存率有关。结论MMP7表达升高与Bcadherin表达降低是胆囊癌恶性生物学指标之一；MMP7与E-cadherin的表达呈负相关；MMP7的表达可以提示胆囊癌患者的预后，并可为胆囊癌患者的抗基质金属蛋白酶治疗提供依据。

Expression and clinical significance of E-cadherin and MMP7 in carcinoma of gallbladder

Purpose To detect the expression of MMP7 and E-cadherin and to evaluate the relationships of their expression with invasion, metastasis and prognosis in carcinoma of gallbladder. Methods Immunohistochemical S-P method was used to analyze the expression of MMP7 and E-cadherin in 63 cases of gallbladder carcinoma. Results The positive rate of MMP7 in carcinoma of gallbladder was prominently increased. MMP7 expression was correlative with TNM stage, liver invasion and lymph node metastasis (P<0.05). The positive rate of E-cadherin was decreased in gallbladder carcinoma. E-cadherin expression was correlative with TNM stage, lymph node metastasis and nerve infiltration (P<0.05). The spearman correlation analysis suggested that the expression of MMP7 and E-cadherin was negatively correlated (P<0.05). The survival analysis indicated that the expession of MMP7, TNM stage and nerve infiltration were associated with survival rate of post-operation. The expression of MMP7 and nerve infiltration may be a prognostic factor of gallbladder carcinoma by Cox regression. Conclusions MMP7 and E-cadherin play important roles in the progression of carcinoma of gallbladder. The expression of MMP7 and E-cadherin in carcinoma of gallbladder has negative correlation. MMP7 may be a useful prognostic marker for patients with advanced gallbladder carcinoma, and its detection may be useful for selecting patients for anti-matrix metalloproteinase therapy.
