部分脾栓塞治疗肝硬化脾功能亢进的临床研究

目的评估部分脾栓塞治疗肝硬化脾功能亢进的临床疗效及价值。方法1988年6月—2004年6月期间对116例肝硬化脾亢患者实施了部分脾栓塞术,观察治疗前后血常规、肝肾和凝血功能,B超及彩色Doppler超声检查明确肝脾大小和门脉系统血流情况,并严密观察病情变化,注意防治并发症。结果本组116例手术均成功(100%),术后随访6~12个月,其血红蛋白含量、白细胞和血小板计数较术前显著增加(P<0.01)并恢复正常,同时其门脉直径、脾血流量和血白蛋白含量较术前改善(P<0.01)。结论部分脾栓塞是治疗肝硬化脾功能亢进的一种疗效较好,有临床应用价值的方法。

Embolic Therapy of Partial Spleen for Hypersplenism Caused by Liver Cirrhosis

Objective To evaluate the cl in ical effect of embolization in partial spleen on hypersplenism caused by liver c irrhosis. Methods Embolization in partial spleen was performed on 116 pat ients with hypersplenism caused by liver cirrhosis from June, 1988 to June, 2004 . Then, some indexes, such as blood routine, hepatic and renal function and bloo d coagulation function, were tested. The sizes of liver and spleen and t he blood flow of portal vein system were detected by type B and Doppler ultrason ic. Results All the 116 patients succeeded in accepting the perform ance. According to 6~12 months' follow-up, their haemoglobin amount and leukocy te and platelet accounts rose significantly when compared with those before oper ation (P<0.01). Not only the above indexes were up to normal, but their port al vein diameters, spleen blood flows and albumin levels in the bloods improved obviously as well when compared with those before (P<0.01). Conclusion Embolic therapy of partial spleen for hypersplenism caused by liver cirrhosis is a clinically valuable way with good effect.
