肝癌射频治疗前后影像学表现的对比研究

目的 观察肝癌射频治疗前后影像学表现,评价肝癌射频治疗效果,寻找有效治疗方法。材料和方法 患者40例,于射频治疗前后采用常规方法行肝脏CT平扫及增强扫描,测正常肝脏及病变部位CT值,观察病变大小及形态改变。经皮股动脉穿刺插管行肝动脉造影。结果 射频治疗前后CT增强与平扫对比,正常组织CT值升高无显著性差异(P>0.05)。肝癌组织射频治疗前,CT增强强化较明显,射频治疗后,治疗区强化较弱,CT值变化之间有显著性差异。治疗后,局部密度呈均匀性降低,与周围组织分界更加清楚。射频治疗前肝动脉造影显示肿瘤区供血丰富,可见肿瘤血管和肿瘤染色。治疗后,肿瘤区供血明显减少。结论 射频治疗肝癌是一种有效的治疗方法,能明显地引起肿瘤组织坏死,特别适用于直径小于5 cm的肿瘤,对于直径大于5 cm的肿瘤,应结合应用其他治疗方法(如介入治疗等),以期提高疗效。

Reseach of CT and Angiography of Hepatic Carcinoma before and after Radio Freqency Treatment

Objective: To observe imageology manifestation of hepatic carcinoma befor and after radio frequency, appraise the effect of treatment and look for the effective method of treatment.Materials and Methods:40 cases,CT scan and enhance scan were done,CT value (Hu) were measured on nornal and invalid position of live, before and after radio frequency. Angiographies of hepatic arteries ware done. Results: Compareing common scan and enhance scan, there is no significant difference(P>0.05) in the rise of CT value in normal tissue of live before and after radio frequency . The tissue of hepatic carcinoma was enhanced obviously before treatment, but was less enhanced after treatment, there is much significant dif-ference( P<0.01) in the change of CT value before and after treatment. After treatment, the density of cancer treated dropped evenly, the boundary between cancer and normal tissue become clear. The blood supply in the cancer region was shown abundantly before treatment but poorly after trea-ment by angiography of hepatic artery. Conclusion: Radio frequency treatment is a effective method to treat hepatic carcinoma, can lead to obvious necrosis of tumor, especially suit to treat the cancer low 5 cm in diameter. For the cancer over 5 cm in diameter, treatment should combine other method, such as interventional treatment, in oder to obtain good result.
