食管癌横向浸润的双对比特征与病理关系研究

目的探索食管癌横向浸润的双对比特征与其癌肿浸润深度的关系。方法对１３８例食管癌患者采用单钡双对比造影法、规定体位、测量方法所获征象与手术、病理作对比。结果（１）癌肿段右侧壁厚度越大，侵入深度越深；（２）癌肿段右缘清晰度与侵入深度有关；（３）癌肿段受累周径越大，侵入越深；（４）癌肿段移动度减少或固定，侵出外膜向周围侵犯明显。结论食管癌横向浸润双对比特征与其癌肿浸润深度有关，并提出了食管癌浸润深度的综合评估指标，对诊断、手术方式选择和预后的评价等有临床应用价值

A Comparative Study Between Double Contrast Signs of Transversal Invasion in Esophageal Carcinoma and Pathology

Objective To investigate the relative between the double contrast signs in esophageal carcinoma and its invasive depth so as to estimate the invasive depth of esophageal carcinoma. Methods Single barium double contrast radiography were taken in 138 cases with esophageal carcinoma by means of routine radiographic position.The figure were measured and compared with the surgical and pathological result. Results (1)The thicker the right border of tumor segment is,the deeper the invasive depth is;(2)The clarity of right border of tumor segment is related to the invasive depth;(3)The bigger circumsference infiltrated by carcinoma it is, the deeper invasive depth is;(4)The decreased esophageal morbility or fixation about tumor segment demonstrated that carcinoma infiltrated through adventitia to the adjacent structure. Conclusion This study suggests that the double contrast signs of transversal invasion in esophageal carcinoma is related to the invasive depth of tumor segment. From these results the comprehensive evaluating criteria about esophageal carcinoma was determined, which is of important clinical value for diagnosis,selection of surgery and estimation of prognosis.
