育龄期女性性腺功能减低60例临床分析

目的探讨育龄期女性性腺功能减低的临床特征。方法对2011年5月-2012年4月收治的60例育龄期女性性腺功能减低患者的临床资料进行回顾性分析。结果 60例女性性腺功能减低患者中,多囊卵巢综合征(PCOS)33例(55.0%),卵巢早衰(POF)10例(16.7%),原发性甲状腺功能减退症(含亚临床甲状腺功能减退症)6例(10.0%),泌乳素瘤5例(8.3%),其他6例(10.0%)。PCOS患者中超质量27例(81.8%);POF患者中FSH40～129.3(83.4±23.3)U/L,E_2 2.1～52.2(26.0±16.2)pmol/L;原发性甲状腺功能减退患者TSH均>100mIU/L,伴T_4、FT_4降低,亚临床甲状腺功能减退患者TSH>5 mIU/L,平均(12.8±2.0)mIU/L;泌乳素瘤患者PRL为89.5～126.9(105.3±12.6)ng/ml。结论育龄期女性性腺功能减低常见原因依次为:PCOS、POF、原发性甲状腺功能减退(含亚临床甲状腺功能减退)、泌乳素瘤等。故在女性性腺功能减低患者中应常规行内分泌检查,提高确诊率。

Clinical analysis 60 cases of childrenbirthing age female gonadal hypofunction

Objective To analyze clinical features of childrenbirthing age female gonadal hypofunction. Methods Collected the clinical data from 60 female in child bearing age in the Department of Endocrinology of Affiliated Hospital of In- ner Mongolia Medical University from May, 2011 to April, 2012 retrospectively. Results Among the 60 patients, there were PCOS of 33 cases, accounting for 55.0% ; POF of 10 cases （ 16.7% ） ; primary hypothyroidism （ including subclinical hypot- hyroidism） of 6 cases （ 10.0% ） ; prolactinoma of 5 cases （ 8.3% ） ; and other 6 cases： 27 cases of PCOS were overweight, accounted for 81.8%. In 10 POF patients, FSH level were 40 - 129.3U/L, the average were（83.4 ± 23.3） U/L, E2 2.1 - 52.2 pmol/L, （26.0 ± 16.2） pmol/L on the average. Primary hypothyroidism with TSH 〉 100 mIU/L, with T4, FT4 de- creased, subclinical hypothyroidism TSH 〉 5 mIU/L, the average was（ 12.8 ± 2. O） mIU/L; In 5 patienis with prolactino- ma, PRL level was 89.5 - 126.9 ng/ml, the average was （ 105.3 _± 12.6） ng/ml. Cohclusion Common successive causes for childrenbirthing age female gonadal hypofunction were including PCOS, POF, primary hypothyroidism （ including subclini- cal hypothyroidism）, prolactinoma. Therefore, most importantly, the female gonadal hypofunction patients should be taken endocrinological examinations routinely so that diagnosis can be clarified as soon as possible.
