不同术式治疗鼻腔鼻窦内翻性乳头状瘤疗效的比较观察

目的比较三种不同手术方式治疗鼻腔鼻窦内翻性乳头状瘤的疗效.方法分别采用鼻侧切开术、改良面中部揭翻径路联合鼻内镜手术及鼻内进路联合鼻内镜手术治疗鼻腔鼻窦内翻性乳头状瘤62例,对其治疗效果进行比较分析.结果经术后随访半年～11年,各组患者症状均消失或减轻.鼻侧切开术36例中复发8例(22.2%),改良面中部揭翻径路联合鼻内镜手术18例中,复发2例(11.1%),鼻内进路联合鼻内镜手术8例中复发1例(12.5%).复发者再次手术后治愈.结论改良面中部揭翻径路联合鼻内镜手术是治疗鼻腔鼻窦内翻性乳头状瘤可靠且有效的方法.

A comparison study on the therapeutic effects of different surgical procedures on inverting papilloma in nasal cavity and paranasal sinus

Objective To compare the therapeutic effects of surgery on inverting papilloma in nasal cavity and paranasal sinus among three kinds of surgical procedures. Methods 62 cases with this kind of lesion were treated by operations respectively,either by lateral rhinotomy,modified midfacial degloving operation combined with endoscopic surgery,or intranasal approach operation combined with endoscopic surgery.Then,the therapeutic effects on the treated cases were comparatively analyzed among these surgical procedures by the end of following up period lasted for half one year to 11 years. Results The reoccurring rate of lesion was 22.2%(8/36)among the cases received the therapy of lateral rhinotomy,it was 11.1%(2/18)among those received the therapy of modified midfacial degloving operation combined with endoscopic surgery and that was 12.5%(1/8)among ones received the therapy of intranasal approach operation combined with endoscopic surgery,while all the symptoms were either disappeared or relieved among the studied 62 cases during the period of following up.However,the cases suffering lesion reoccurring were cured by re-operating therapy. Conclusions The therapy of modified midfacial degloving operation combined with endoscopic surgery is more effective for the treatment of inverting papilloma in nasal cavity and paranasal sinus than other kinds of surgical procedure.
