门静脉高压症断流术后门静脉血栓形成的原因及预防

目的探讨门静脉高压症断流术后门静脉血栓形成的原因和治疗方法。方法对1989～1999年168例施行断流术的病例进行回顾性分析。结果全组总死亡率2.4％(4/168),门静脉血栓形成70例,并同时都伴发脾静脉内血栓。1例并发小肠梗死,抗血栓药物应用时间4～8天,平均5.8天。结论切脾前不先结扎脾动脉和围手术期禁用止血药,是预防门静脉血栓形成的关键,手术后早期应用小剂量肝素和扩容、活血药物,对门静脉血栓可起到有效的预防和治疗作用。

The cause and prevention of formation of portal vein thrombosis alter portal hypertension portaiazygons disconnection.

Objective To investigate the canse and therapeutic methods of the formation of portal vein thrombisis after portal hypertension porta- azygons disconnection. Method retrospective analysis of 168 cases with porta- azygons disconnection from 1989 to 1999 was conducted.Result Total mortaligy was 2.4% (4/168) portal vein thrombosis complicated by splentic vein thrombosis was developed in 70 cases small intestine infarction was developed in one case.The antithrombo-sis drug was adminstratered for 4 - 8 days wirh average length of 5.8 days. Conclusion rigaton of splenic vein after spleen resection and haemostatic drug forbidden in perioperative period are the key to prevent portal vein thrombosis.The application of small dosage of heparin and dilatation medicine of actiuate blood, would play an important roles in prevention and treatment for thrombosis.
