初诊2型糖尿病患者性别及年龄对心率变异性的影响

摘    要:目的探讨初诊2型糖尿病患者的性别及年龄对心率变异性(HRV)时域指标的影响。方法初诊2型糖尿病患者391名,男183例,女208例,年龄40~70岁。分为40岁~、50岁~、60岁~年龄组。24 h动态心电图检查,测定心率变异性时域指标:全部正常窦性R-R间期值的标准差(SDNN);全程按5 min分成连续的时间段,先计算每5 min正常R-R间期的平均值,再计算所有平均值的标准差(SDANN);全程按5 min分成连续的时间段,先计算每5min正常R-R间期的标准差,再计算所有标准差的平均值(SDNNI)。采用SPSS 10.0软件进行统计学分析。结果初诊2型糖尿病患者SDNN、SDANN、SDNNI男性高于女性,在40岁~年龄组和50岁~年龄组中差别有统计学意义(P<0.01),在60岁~年龄组中差别无统计学意义。女性年龄与SDNN(r=-0.374)、SDANN(r=-0.319)、SDNNI(r=-0.271)呈负相关,差别有统计学意义(P<0.01)。男性年龄与SDNN(r=-0.273)、SDANN(r=-0.293)、SDNNI(r=-0.369)、全部相邻R-R间期差的均方根(rMSSD,r=-0.317)、24 h内差值超过50 ms的连续正常获得R-R间期数所占的百分数(PNN50,r=-0.264)呈负相关,差别有统计学意义(P<0.01)。结论初诊2型糖尿病患者的性别和年龄与心率变异性时域指标经统计学分析,差别有统计学意义。

Abstract:Objective To know whether the gender and age have effects on the variability of heart rates(HRV) among newly diagnosed type 2 diabetic subjects.Methods We examined the time domain indices of HRV by every 5 minutes with 24 h Holter recording in 391 newly diagnosed patients with type 2 diabetes.Results HRV time domain parameters(SDNN,SDANN,SDNNI) were especially impaired in diabetic women compared with diabetic men under 60 years old.SDNN(r=-0.374,P<0.01),SDANN(r=-0.319,P<0.01),SDNNI(r=-0.271,P<0.01) were negatively related to aging in female.SDNN(r=-0.273,P<0.01),SDANN(r=-0.293,P<0.01),SDNNI(r=-0.369,P<0.01),rMSSD(r=-0.317,P<0.01),PNN50(r=-0.264,P<0.01) were negatively related to aging in male.Conclusion Newly diagnosed type 2 diabetes has an age and gender-specific impact on the HRV.
