我国南方社区人群高甘油三酯血症腰围身高比表型与慢性肾脏病的关系

摘    要:目的 探讨我国南方社区人群中高甘油三酯血症腰围身高比表型(HWHtR)与慢性肾脏病(CKD)之间的关系.方法 于2012年6月~10月在珠海市进行慢性肾脏病的横断面调查.HWHtR表型被定义为腰高比(WHtR)≥0.55并且血清甘油三脂≥2.0 mmol/L.参考HWHtR诊断标准将参与者分为HWHtR组与non-HWHtR组.慢性肾脏病(CKD)诊断标准:eGFR<60 mL/(min.1.73 m2)或者ACR≥30 mg/g.建立Logistic回归模型探讨慢性肾脏病与HWHtR表型之间的关系.结果 与non-HWHtR组人群比较,HWHtR组参与者,慢性肾脏病患病率更高(11.1% vs 33.0%,P<0.001).进一步建立Logistic回归模型探讨慢性肾脏病与HWHtR表型之间的关系.未调整模型中,HWHtR与CKD密切相关(OR=3.23,95% CI 2.32-4.48,P<0.001).进一步调整年龄、性别、高血压病史、糖尿病病史、收缩压、舒张压、吸烟、饮酒、缺乏体育锻炼和文化程度,HWHtR与CKD仍密切相关(OR=2.36,95% CI 1.52-3.67,P<0.001).进一步调整BMI,HWHtR与CKD的关系仍较密切(OR=2.12,95% CI 1.34-3.35,P<0.001).结论 在我国南方社区人群中高甘油三酯血症腰围身高比表型与CKD存在相关关系.

Abstract:Objective To investigate the relationship between hypertriglyceridemic waist-to-height ratio phenotype (HWHtR) and chronic kidney disease (CKD) in a community population in South China. Methods A cross-sectional study was conducted among 2142 residents in Zhuhai (Guangdong Province, China) from June to October of 2012. The HWHtR phenotype was defined as a waist-to-height ratio(WHtR) ≥0.55 and triglyceride level ≥2.0 mmol/L, based on which the participants were divided into HWHtR group and non-HWHtR group.CKD was defined as an eGFR<60 mL/min per 1.73 m2 or an ACR ≥30 mg/g. A logistic regression model was established to investigate the relationship between chronic kidney disease and HWHtR phenotype.Results Compared with the non-HWHtR group,the HWHtR group had a higher prevalence of chronic kidney disease (11.1% vs 33%, P<0.001). Analysis using the logistic regression model showed that HWHtR was significantly associated with CKD in the unadjusted analyses(OR=3.23,95% CI:2.32-4.48,P<0.001).After adjustment for age, sex,history of hypertension,history of diabetes,systolic blood pressure,diastolic blood pressure,drinking,physical exercise, education and current smoking, HWHtR was significantly associated with CKD (OR=2.36, 95% CI: 1.52-3.67, P<0.001); the association of HWHtR and CKD was still significant after further adjustment for BMI (OR=2.12, 95% CI: 1.34-3.35, P<0.001). Conclusion Our finding suggests that HWHtR is associated with CKD in this community population.
