彩色多普勒超声诊断下肢深静脉血栓形成的价值探讨

目的探讨彩色多普勒超声对下肢深静脉血栓（DVT）的诊断价值。方法对131例下肢DVT患者的声像图表现特点进行回顾性分析。结果急性下肢DVT表现为管径增宽，内充满较均匀的低回声，加压后血管不能被压闭，CDFI示无彩色血流信号或彩色血流信号充盈缺损：亚急性下肢DVT表现为增宽的管腔回缩。腔内实质性回声相对增强，附着于管壁，管壁规整，CDFI示血栓周边可见细点状或细线状血流信号；慢性下肢DVT血栓发生部位管腔狭窄，血栓呈条索状较高回声，紧密附着于管壁使管壁呈不规则增厚，CDFI可探及不同程度的血流信号。结论彩色多普勒超声是检查下肢DVT的最佳方法。

Value of color Doppler flowing imaging in diagnosis of deep venous thrombosis in lower limbs

Objective To explore the value of color Doppler flowing imaging in the diagnosis of deep venous thrombosis(DVT) in lower limbs.Methods A total of 131 patients with DVT were reviewed and analyzed in the color Doppler ultrasonographic features.Results For the acute DVT lower extremity,the conventional ultrasound could display an evenly echogenic structure within the lumen of the vein,the distension of the involved vein,and loss of normal venous compressibility when slight pressure was exerted on the skin overlying the vein.The color Doppler flowing signals were not detected or weakly visualized in venous lumen of obstructing thrombus.The subacute DVT eases had distended venous lumen,with stronger echogenic structure which neighbored closely to the whole vessel wall.A kind of small dots or tiny strip of color Doppler signal could be detected around the thrombus.The chronic DVT cased had a shortened venous lumen near the thrombus and striped like enhanced echogenic signal,closed related the vessel wail,and made the wall irregular factor.The CDEI could detect changeable signal.Conclusion Color Doppler ultrasonography is the best method in diagnosis of Deep Venous thrombosis in lower limbers.
