摘    要:右美托咪定是高效和高选择性的α2肾上腺素受体激动剂,通过激活中枢神经系统和外周脊髓的α2肾上腺素受体而产生抗交感、镇静和镇痛的作用。随着对右美托咪定研究的逐渐深入,大量资料证实其对于于吸入麻醉药引起的神经损伤、创伤引起的神经损伤、缺血再灌注引起的神经损伤以及外源性谷氨酸诱导的神经损伤均具有保护作用。文章从右美托咪定抑制中枢和外周儿茶酚胺的释放、抑制神经细胞的凋亡、抑制氧化作用、抗神经炎症反应、作用于咪唑啉I受体等机制对其神经保护作用进行了综述,为其基础研究和临床应用提供依据。

Abstract:Dexmedetomidine is an efficient and highly selectiveα2 adrenergic receptor agonist,it has sympathetic blocking,sedative and analgesic effects by activating theα2 adrenergic receptor in the central nervous system and spinal cord.With the increase of clinical application of dexmedetomidine,A large number of data have confirmed its protective effect on nerve injury caused by inhaled anesthetics,nerve injury caused by trauma,nerve injury caused by ischemia-reperfusion and nerve injury induced by exogenous glutamate.This paper reviews the neuroprotective mechanisms of dexmedetomidine,including inhibition of catecholamine release,neurotrophic nerve,inhibition of neuronal apoptosis,antioxidant effect,inhibition of neuroinflammatory response,imidazoline I receptor participation,inhibition of calcium ion influx and release of glutamate,it provides the basis for its basic research and clinical application.
