血清25-羟维生素D3水平与婴儿特应性皮炎严重度的相关性研究

摘    要:目的探讨血清25-羟维生素D3(25-(OH)D3)水平与婴儿特应性皮炎严重度的相关性,以进一步探究25-(OH)D3与特应性皮炎的关系。方法选取160例特应性皮炎患儿,收集患儿的相关资料并检测其血清25-(OH)D3、嗜酸性粒细胞比例的水平。对比轻中度特应性皮炎患儿和重度特应性皮炎患儿的血清25-(OH)D3、嗜酸性粒细胞比例水平。分析特应性皮炎患儿25-(OH)D3缺乏的危险因素以及血清25-(OH)D3与SCORAD评分、嗜酸性粒细胞比例的相关性。结果重度特应性皮炎患儿平均月龄为10.01±6.64个月,25-(OH)D3为46.46±18.91nmol/L,显著低于轻中度特应性皮炎患儿,而嗜酸性粒细胞比例为0.125±0.039,显著高于轻中度特应性皮炎患儿(P<0.05)。喂养方式为特应性皮炎患儿25-(OH)D3缺乏的危险因素,其中,母乳喂养患儿发生25-(OH)D3缺乏的风险显著高于人工喂养患儿(P<0.05);补充25-(OH)D3是特应性皮炎患儿25-(OH)D3缺乏的保护性因素(P<0.05)。特应性皮炎患儿血清25-(OH)D3与SCORAD评分、嗜酸性粒细胞比例均呈负相关关系(r=-0.371、-0.342,P<0.05)。结论血清25-(OH)D3水平降低与婴儿特应性皮炎的严重程度相关,临床应重视患儿血清25-(OH)D3水平的检测并积极防治25-(OH)D3缺乏以提高婴儿特应性皮炎的防治效果。

Abstract:Objective To investigate the relationship between serum 25-hydroxyvitamin D3 level and the severity of infantile atopic dermatitis,and to further explore the relationship between 25-hydroxyvitamin D3 and atopic dermatitis.Methods 160 children with atopic dermatitis were enrolled.The relevant data of the children were collected and the serum levels of 25-(OH)D3,serum total IgE and eosinophils were measured.Serum 25-(OH)D3,and eosinophil levels were compared between children with moderate atopic dermatitis and children with severe atopic dermatitis.To analyze the risk factors of 25-hydroxyvitamin D3 deficiency in children with atopic dermatitis and the correlation between serum 25-hydroxyvitamin D3 and SCORAD score,serum total IgE,and eosinophil ratio.Results The average age of children with severe specific dermatitis was(10.01±6.64)months,and that of 25-(OH)D3 was(46.46±18.91)nmol/L,which was significantly lower than that of children with moderate atopic dermatitis.The ratio of acidic granulocytes was(0.125±0.039),which was significantly higher than that of children with moderate atopic dermatitis(P<0.05).The feeding method was a risk factor for 25-hydroxyvitamin D3 deficiency in children with atopic dermatitis.Among them,the risk of 25-hydroxyvitamin D3 deficiency was significantly higher in breast-fed children than in artificial feeding(P<0.05);Hydroxyvitamin D3 was a protective factor for 25-hydroxyvitamin D3 deficiency in children with atopic dermatitis(P<0.05).Serum 25-hydroxyvitamin D3 was negatively correlated with SCORAD score and eosinophil ratio in children with atopic dermatitis(r=-0.371,-0.342,P<0.05).Conclusion The decrease of serum 25-hydroxyvitamin D3 level is related to the severity of atopic dermatitis in infants.The clinical value of serum 25-hydroxyvitamin D3 should be monitored and the 25-hydroxyvitamin D3 deficiency should be actively prevented to improve infant atopy.The prevention and treatment effect of dermatitis.
