儿童癫癎860例临床特点分析

目的 分析儿童癫癎的临床特点和预后因素.方法 对2009年6月至2012年6月贵州省人民医院的860例儿童癫癎患儿进行回顾性分析,对病史资料、临床表现、脑电图、抗癫癎药物的应用情况及随访结果进行统计分析.结果 能明确病因者有158例（18.4％）,其中,产前、产时及围生期因素94例（10.9％）;中枢神经系统感染10例（1.2％）.儿童癫癎无明显诱因发作为645例（75.0％）,其次为疾病诱发140例（16.3％）;发作频率＜5次/d为773例（90.1％）,发作持续时间1～2 min为529例（62.0％）,以四肢抖动550例（64.0％）、双眼凝视391例（45.5％）、呼之不应571例（66.4％）、四肢强直145例（16.9％）症状多见;局灶性发作296例（59.8％）,其中复杂局灶性发作占56例（11.3％）.结论 儿童癫癎在临床上表现复杂,良好的预后需提高依从性.

Clinical features of 860 cases of epilepsy in children

Objective To analyze the clinical manifestation of 860 cases epilepsy in children.Methods During 2009 June and 2012 June,clinical data of 860 epilepsy in children patients was analyzed retrospectively.According to the history data,clinical manifestations,electroencephalography,application of antiepileptic drugs and follow-up results were analyzed.Results There were 158 （18.4%） patients who had definite etiology prenatal; intrapartum and perinatal factors were the most common causes of structural-metabolic epilepsy （94cases,10.9%） ; the infection of central nervous system was 10cases （1.2%） of all postnatal factors.Seizure frequency less than five times a day was 90.1% （773/858） ; lasting time of seizures 1-2 minutes was 62.0% （529/860） ; limb jitter was 64.0% （550/860） ; eyes stare was 45.5% （391/860） ;unresponsive was 66.4% （571/860） ; rigidity of limbs was 16.9% （145/860）.Oocal seizure was 59.8% （296/495）,which accounted for 11.3% （56/495）of complex focal seizures.Conclusion The clinical manifestations of epilepsy in children are complicated.
