153例复杂肝内胆管结石病人围手术期的护理与观察

目的总结复杂肝内胆管结石病围手术期的护理要点及经验。方法回顾性分析1998-2005年我院收治复杂肝内胆管结石153例的临床与护理资料,统计分析血浆管引流量、术后并发症,残石率及护理情况。结果本组病人结石残留32例,残石率为21.0%,术后发生并发症37例,占24.2%,术后3d血浆管平均引流量50ml。153例复杂肝内胆管结石病人术前均做好心理护理及检查护理,加强术后“T”管、血浆引流管观察与护理。结论严密护理和观察是手术成功的基础;有效的健康教育是减少结石复发,提高治疗效果的保证,及时观察病情,加强术后“T”管、血浆引流管观察与护理是防止和早发现并发症发生的关键。

Nursing of 153 cases with complex intrahepatic biliary calculi during perioperation

Objective To summarize the nursing essentials and the nursing experiences of 153 cases with complex intrahepatic biliary calculi during perioperation. Methods The clinical and nursing information of 153 cases with complex intrahepatic biliary calculi hospitalized in our hospital from 1998 to 2005 were analyzed retrospectively. The drainage amount of plasma tubes, the postoperative complications, the rate of residual stone and the nursing conditions were analyzed. Results 32 cases had residual stones with the rate of residual stone of 21.0%, 37 cases had postoperative complications accounting for 24.2%, and the mean drainage amount of 3d plasma tubes was 50ml. 153 cases with complex intrahepatic biliary calculi were given good psychological nursing and examination nursing before the operation. The observation and nursing of T tubes and drainage tubes of plasma were strengthened after the operation. Conclusions Strict nursing and observation are the basis for the successful operation, effective health education is a guarantee for reducing the recurrence of calculi and improving the treatment results. In addition, timely observing the diseases conditions, strengthening the observation and nursing of postoperative T tube and plasma drainage tube are the key points for preventing from and early finding the complications.
