断脐时间与新生儿出生Apgar评分的关系探讨

目的:探讨新生儿娩出至结扎脐带时间长短与新生儿出生Apgar评分的关系。方法:对536例新生儿娩出至结扎脐带按时间不同分为A组(<1min)、B组(≥1min)。分别观察两组新生儿出生后1min、5min Apgar评分中五项指标情况(即心率、呼吸、肌张力、喉反射和皮肤颜色的得分)。结果:新生儿出生1min Apgar评分五项指标中,呼吸和皮肤颜色两项指标的评分A组高于B组(P<0.01);出生5min Apgar评分五项指标两组间差异均无统计学意义(P>0.05)。但B组新生儿Ap-gar低评分(Apgar评分≤7分)发生率高于A组(P<0.05)。结论:正常足月新生儿娩出后结扎脐带以1min内完成较合适。娩出至结扎脐带时间过长会增加新生儿出生Apgar低评分(新生儿窒息)的发生率。

Relationship between umbilical cord cutting time and Apgar score in newborns

Objective:To explore the relationship between different umbilical cord cutting time and Apgar score.Methods:According to umbilical cord cutting time after birth,536 neonates were divided into group A (<1 minite) and group B (≥1 minite).Apgar scores (heart rate,breathing, muscular tension, laryngeal reflex, skin colours) were calculated on 1 minite and 5 minites after birth.Results:The scores of breathing and skin colours in group A were higher than those in group B (P<0.01) on 1 minite after birth,but there was no difference in Apgar scores between two groups on 5 minites after birth.The incidence of lower Apgar scores (total scores ≤7) in group A was higher than that in group B.Conclusion:The proper umbilical cord cutting time is within 1 minute after birth,and longer umbilical cord cutting time increases incidence of low Apagar scores in newborns.
