摘    要:白内障术后晶状体后囊膜混浊(posteriorcapsularopacification,PCO)(后发性自内障)是现代白内障术后最常见的并发症,防治PCO是保障白内障手术远期疗效的关键。术中残留的晶状体上皮细胞增生、移行和上皮一间质转化是PCO形成的细胞学基础。转化生长因子p(transforminggrowthfactorbeta,TGF一8)通过正性调控晶状体上皮细胞的增生、迁移、转分化及凋亡等,促进了PCO的发生发展过程。设想可通过对TGF—B、TGF—p受体及其下游通路中的Smad蛋白和基质金属蛋白酶等进行干预来防治PCO。采用结膜囊涂抹及前房注射TGF—B抗体、基质金属蛋白酶抑制剂及结缔组织生长因子反义寡核苷酸或特异性抗体等将成为PCO防治的新途径。

Abstract:Posterior capsular opacification (PCO) is a most common complication of modern cata- ract surgery. The prevention of PCO is the key to ensure long-term efficacy of cataract surgery. The prolifera- tion, migration, and epithelial - mesenchymal transition of intraoperative residual lens epithelial cells are cellular basis of PCO. Transforming growth factor [3 (TGF-13)participates in the development of PCO by regu- lating lens epithelial cells proliferation, migration, differentiation and apoptosis. It is envisaged that the reg- ulation of TGF-~, TGF-~ receptor and its downstream pathway ( Smad proteins and matrix metalloproteinas- es) can prevent PCO. By the means of conjunctival sac daub and anterior chamber injection, TGF-[3 antibod- ies, matrix metalloproteinase inhibitors and connective tissue growth factor antisense oligonucleotides or specif- ic antibodies may become the new ways of preventing PCO.
