摘    要:目的 探讨显微镜观察药物敏感性检测(MODS)方法用于结核分枝杆菌(MTB)分离培养和耐药检测的应用效果.方法 收集276份肺结核临床疑似患者的痰标本,用MGIT 960 TB、罗氏法、MODS法检测MTB.用MODS法对80株MTB临床分离株进行药敏检测,并将结果与传统罗氏法进行比较.结果 MODS技术直接检测痰标本的阳性率为46%,阳性检出时间中位数为9 d(7~12 d).MODS检测链霉素、异烟肼、利福平、乙胺丁醇药敏结果与传统罗氏法符合率分别为95.0%、96.25%、97.5%、85.0%,检测敏感度分别为83.3%、95.65%、96.15%、70.83%,特异度分别为98.3%、96.50%、98.15%、91.07%.结论 MODS方法是一种快速简便、费用低、准确的诊断和药敏检测方法,具有广泛的应用前景.

Abstract:Objective To investigate the application effect of microscopic observation drug susceptibility (MODS) assay on mycobacterium tuberculosis (MTB) culture and detection of drug-resistance.Methods MTB in 276 consecutive sputum samples taken from suspected patients with tuberculosis was tested by using MGIT 960 TB,L-J culture and MODS assay,and then we used MODS assay to detect drug susceptibility of MTB in 80 strains of clinical isolates and compared the results with that of the L-J.Results The detection rate of 276 sputum samples collected from TB suspects by MODS assay was 46%,and the median was 9 days (7-12 d).The coincidence rates of the susceptibility to streptomycin,isoniazid,rifampicin and ethambutol detected by MODS and L-J were 95.0%,96.25%,97.5% and 85.0%,respectively; the sensitivities of MODS method were 83.3%,95.65,96.15% and 70.83%,and the specificities were 98.3%,96.50%,98.15% and 91.07%,respectively.Condusion MODS assay is a simple,low-cost,accurate diagnosis and drug sensitivity test method,which is appropriate for diagnosis of TB.
