多发性骨髓瘤临床诊断体会

目的探讨多发性骨髓瘤(MM)的临床表现及实验室检查特点,为早期诊断提供依据。方法对2009年1月—2011年11月该院收治的15例多发性骨髓瘤进行回顾性分析,收集整理其临床资料和有关实验室检查结果,分析其发病特点以及实验室检查特点。结果 15例患者中,男8例,女7例,年龄>50岁13例,误诊为巨幼细胞性贫血2例、肾脏疾病1例、骨关节疾病5例、感染性疾病2例、急性胃炎1例、五官科疾病1例、结缔组织病1例、颅内占位1例、眼科疾病1例。结论临床中应加强对多发性骨髓瘤特征的认识,掌握疾病特点,详细询问病史,进行认真细致的体格检查,以减少或防止发生漏诊误诊。

Diagnosis reflections on clinical of multiple myeloma

Objective To investigate the clinical and laboratory characteristics of muhiple myeloma（MM） and to provide basis for early diagnosis of MM. Methods Fifteen cases of MM in our hospital during Jan. 2009 to Nov 2011 were analyzed retrospectively. Clinical features,initial presentation, and laboratory tests were analyzed. Results Among 15 cases of preliminary diagnosis MM,8 cases men and 7 cases women, 13 cases were over 50 years old. Main diseases being misdiagnosed were 2 cases Nutritious megaloblastic anemia, 1 case kidney diseases ,5 cases bone and joint diseases, 2 cases infection diseases, 1 case acute gastritis, 1 case nose disease, 1 case connective tissue disease, 1 case tumor in brain, 1 case eye disease. Conclusion We must strengthen overall recognition for characteristics of MM, know well characteristics of diseases, inquire detailed case history, make micromesh medical examination to reduce or prevent misdiagnosis.
