胆囊结石合并胆总管结石的腹腔镜术式选择

目的:探讨胆囊结石合并胆总管结石行腹腔镜手术的术式选择。方法:回顾分析为103例胆囊结石合并胆总管结石患者行腹腔镜手术的临床资料。结果:103例手术均顺利完成,手术时间45～180 min;19例经胆囊管完成胆总管探查取石,84例行胆总管切开探查术,术后住院3～8 d,4例发生胆漏。结论:胆囊结石合并胆总管结石的术式选择应根据患者具体情况,制定最适合的个性化治疗方案,真正体现以患者为中心的治疗模式。

Selection of laparoscopic operation in treatment of cholecystolithiasis and choledocholithiasis

Objective:To evaluate the choice of laparoscopic operation methods in treatment of cholecystolithiasis and choledocholithiasis.Methods:The clinical data of 103 cases of cholecystolithiasis and choledocholithiasis treated by laparoscopy was retrospectively analyzed.Results:All the operations were performed successfully.The operation time was 45-180 min.There were 19 cases of transcystic duct common bile duct exploration and 84 cases of common bile duct exploration.The postoperative hospital stay was 3-8 d.Bile leakage took place in 4 cases.Conclusions:Choice of operation methods in treatment of cholecystolithiasis and choledocholithiasis should be based on patient’s specific condition.Individualized treatment is the most suitable for patients.This is the patient-centered treatment modality.
