实验性急性心肌梗塞的酶组织化学

摘    要:取狗8只,缩扎左冠状动脉前降支20 min后,分别取缩扎上、下方的心肌组织,冰冻切片。6只以脱氢酶染色,2只为对照,观察切片发现缩扎上、下方的心肌纵切面上,有酶染蓝色颗粒聚积在闰盘两侧呈“竹节”样,部分心肌有扭曲、断裂,横切面上心肌酶染颗粒不均匀。这皆说明冠状动脉缺血初期伴明显反射性心肌收缩。小部分心肌可见变色反应,说明心肌缺血中的渐进性坏死过程。所以,心肌梗塞早期必伴动力型心肌梗塞(DMI)及静力型心肌梗塞(SMI)两个过程。而心绞痛发生似乎以动力型心肌变化为主,故在决定防治上应全面考虑。

Abstract:The morphology of myooardial autodistruotion caused by excessive non-physiologioal over-contraction and stretching in experimental myooardial infarction in animals was discovered by Kanedo and named dynamic myocardial infarction (DMI), in differentiation from static isohemio myooardial infarction (SMI). This discovery is of great significance in the prevention and treatment of the disease. In 8 dogs, the anterior descending branches of the left coronary artery were ligated. After 20 minutes, myooardial tissue, proximal and distal to the ligature, were taken separately and compared with corresponding sections of 2 normal dogs. After quick freezing and staining with 6 dehydroenzymes. Eesults show that in longitudinal myooardial sections, enzyme-stained blue granules in mitochondria showed segmented "bamboo like" accumulation on both sides of the intercalate disc. In transverse sections, the granules were unevenly scattered. Parts of the myocardium showed twisting and breakage. These changes prove that the initial phase of acute myocardial infarction (AMI) is accompanied by obvious reflex myooardial contractions. A small part of the myocardium may display discoloration, indicates a gradual necrosing process of the isohemio myocardium.The conclusion is that in early AMI the two processes mentioned above must exist, but angina peotoris seems to be dominated by dynamic myooardial change. Therefore, overall consideration is necessary in its prevention and treatment.
