3种肿瘤标志物联合检测对胆管癌诊断的临床意义

目的探讨胆管癌患者糖类抗原19-9(CA19-9)、糖类抗原125(CA125)和细胞角蛋白19(CK19)的血清学变化及临床意义。方法采用电化学发光免疫分析法(ECLIA)检测54例胆管癌患者(观察组)和42例健康体检者(对照组)CA19-9、CA125及CK19的血清学水平,并计算3种标志物单项以及联合检测在胆管癌诊断中的敏感性、特异性及准确性。结果血清CA19-9和CK19水平诊断胆管癌的敏感性较好(分别为81.48%和92.59%),血清CA125的特异度(61.29%)最好,3种标志物联合检测的准确性为92.07%,特异性为92.59%,敏感性为87.04%。结论肿瘤标志物联合检测对胆管癌的临床诊断有协同和优势互补的作用,可使准确性增加,误诊率降低。

Clinical significance of combined detection of CA19-9,CA125 and CK19 in diagnosing cholangiocarcinom

Objective To explore the serum concentration variation of CA19-9,CA125 and CK19 in cholangiocarcinom and its clinical significance.Methods The electrochemiluminescence immunoassay(ECLIA) was adopted to detect serum CA19-9,CA125 and CK19 levels in 54 patients with cholangiocarcinoma and 42 healthy people with physical examination.The sensitivity,specificity and accuracy of the single and combined detection of these 3 markers were calculated.Results Serum CA19-9 and CK19 had better sensitivity in diagnosing cholangiocarcinoma,and the rates were 81.48% and 92.59%,respectively.Serum CA125 had the best specificity(61.29%).The accuracy of combined detection of 3 markers was 92.07%,the specificity was 92.59%.The misdiagnosis rate was decreased to 87.04%.Conclusion The combined detection of tumor markers has synergic action and plays the role of complementary advantages in the diagnosis of cholangiocarcinoma,which can increase the diagnostic accuracy and reduce the misdiagnosis rate.
