急性重症胰腺炎非手术治疗104例诊治体会

目的:探讨分析急性重症胰腺炎非手术治疗的效果和中转手术的指征.方法: 回顾分析我院2000年9月至2007年12月对104例急性重症胰腺炎采用非手术治疗的治疗措施及治疗效果.结果:104例中治愈86例(82.6%),死亡10例(9.6%),中转外科手术8例(7.6%),出现并发症24例(23.0%).结论:急性重症胰腺炎采用积极的非手术治疗可获得满意疗效,中转手术的指征为胰腺坏死组织继发感染、不能控制的胆道感染及治疗期间出现其他外科并发症.

Nonoperative Treatment in 104 Patients with Severe Acute Pancreatitis

Objective:To investigate the efficacy of nonoperative management and indication of conversive surgical intervention of severe acute pancreatitis(SAP).Methods:From Sep.2000 to Dec.2007,104 patients with SAP were treated by nonoperative management in our hospital.The mortality,complications and indication of conversive operation of these patients were retrospectively studied.Results:86 patients of 104 cases with SAP were cured(82.6%)and 10 patients(9.6%)died.There were 24 cases(23.0%) developed various complications in the course of management.Eight patients(7.6%)were converted to surgical intervention.Conclusion:The efficacy of nonoperative management is satisfactory in patients with SAP.If the patient developes pancreatic infection,uncontrolable biliary infection or surgical complications,the surgical intervention must be undergo in time.
